Zusammenfassung
Bei den Anfällen des sogenannten Carotis-Sinus-Syndroms handelt es sich um spontan auftretende oder wie beim hyperaktiven Carotis-Sinus-Reflex-Syndrom um durch Druck auf die Carotis-Gabel am Hals ausgelöste reflektorische Ohnmachten bei disponierten Personen.
Diesem hyperaktiven Carotis-Sinus-Syndrom liegt nachWeiss und Mitarbeitern ein komplexes Reflexgeschehen zugrunde (1. vagal-cardialer Typ, 2. depressorischer Typ, 3. cerebraler Typ).
In dem Beobachtungsgut des Autors stehen 24 spontane Carotis-Sinus-Syndrome 427 latenten Carotis-Sinus-Reflex-Synkopen bei disponierten, meist älteren arteriosklerotischen Personen meistens mit dem cardialen Typ des Reflexgeschehens gegenüber.
Nach der Schilderung der klinischen Bedeutung des hyperaktiven Carotis-Sinus-Syndroms wird über das Vorkommen der cardial-vagalen Reflexohnmacht in einem größeren Beobachtungskreis berichtet.
Im einzelnen beschreibt der Autor die Auswirkung des hypersensitiven Carotis-Sinus-Syndroms cardialen Typs auf die klinische Symptomatik (Bewußtsein, Hautfarbe und Hautwärme), auf das EKG, das EEG und auf den arteriellen und venösen Blutdruck.
Kymographische Herzuntersuchungen und vorsichtige Herzkatheterprüfungen während der asystolischen Phase des Carotis-Sinus-Syndroms cardialen Typs geben Hinweise über die Herzdynamik des Syndroms.
Die pathologisch-physiologische und klinische Analyse des Carotis-Sinus-Syndroms cardialen Typs deckt die zugrunde liegende pathogenetische Gesetzmäßigkeit auf.
Summary
Attacks of the so-called carotid sinus syndrome may occur spontaneously or as in the hyperactive carotid sinus reflex syndrome they may be caused by pressure on the carotid sinus of the neck via a reflex syncope in predisposed persons.
According toWeiss and coworkers the hyperactive carotid sinus syndrome is based on a rather complex reflectoric mechanism (1. vagal-cardiac type, 2. depressor type, 3. cerebral type). The author observed 24 cases of the spontaneously occurring carotid sinus syndrome and 427 cases of latent carotid sinus reflex syncope in mostly older patients with arteriosclerosis showing mainly the cardiac type of the reflex mechanism.
After discussing the clinical importance of the hyperactive carotid sinus syndrome the author reports about the occurrence of the cardiac-vagal reflex syncope in a larger group of patients. In particular he discusses the effect of the hypersensitive carotid sinus syndrome of the cardiac type on the clinical symptomatology (state of consciousness, color of the skin, and skin temperature) on the ECG, the EEG and on the arterial and venous blood pressure. Kymographic cardiac examinations and cautious heart catheterizations during the asystolic phase of the carotid sinus syndrome of the cardiac type give further information about the cardiac dynamics of the syndrome.
The physiologic-pathologic and clinical analysis of the carotid sinus syndrome of the cardiac type explains the basic pathogenetic regulations.
Résumé
Dans les attaques du syndrome sino-carotidien il s'agit de syncopes qui s'évoluent spontanément. Dans le syndrome hyperactive des réflexes sino-carotidiens il s'agit de syncopes provoquées par action des réflexes à la région de la division des carotides du cou chez personnes disposées.
SelonWeiss et collaborateurs la base de ce syndrom hyperactive sino-carotidien est très complexe (1. type cardio-vagal, 2. type dépresseur, 3. type cérébrale).
Dans le matériel de l'auteur se trouvent 24 cas du syndrome sino-carotidien spontané et 427 syncopes latentes sino-carotidiennes chez les malades disposés pour la plupart chez les malades à l'âge avancé atteints de l'athérosclerose montrant souvent de type cardial du réflexe sino-carotidien hyperactive.
Après la description de l'importance clinique du syndrome hyperactive sinocarotidien l'auteur rapporte l'apparition des syncopes cardiovagales à cause d'un grand nombre des malades. Dans des cas isolés l'auteur décrit l'effet du réflexe sino-carotidien du type cardial dans la symptomatologie clinique (conscience, couleur de la peau, chaleur de la peau), les effets à l'EKG, et à l'EEG et les effets à la tension artérielle et veineuse. Des examens kymographiques et des examens de cathéterisation du coeur pratiqués avec précaution fournissent des dates de l'action cardiale pendant la phase asystolique du type cardiale de ce syndrome. L'analyse pathophysiologique et clinique du syndrome sinocarotidien du type cardial révèle les règles fondamentales pathogéniques.
