Summary
Traumatic ruptures and aneurysms of the thoracic aorta have increasingly been observed during the last decade. According to the literature and our own experience based on 23 injured people, a total of 305 patients have hitherto been treated surgically. 87 patients were operated upon within the early phase (rupture) with a lethality rate of 24%; 218 patients underwent surgery after an aneurysm had developed — the lethality rate in this case was 7%.
Pathology, symptomatology, diagnosis, operative methods and operative aids and appliance are discussed. We think the best method to repair such a lesion is a direct suture, and end-to-end-anastomosis, or the implantation of a dacron prothesis after the performance of an atriofemoral bypass and heparinization. Simpler methods e.g. the external shunt are described for emergency cases with multiple injuries of other parts of the body.
We observed six patients with acute rupture and 17 patients with posttraumatic aneurysms. Operations were performed on four patients with rupture 5 h, 6, 8 and 18 days after the trauma. One woman died before the operation could even be started from a perforation of the hematoma into the esophagus. Another woman died 2 h after the accident from various other severe injuries. Of the 17 operated patients with traumatic aneurysms one man died 5 months after the operation from paraplegia. The remaining 16 patients are now, up to 12 years after the surgical intervention, without complaints (symptoms) and capable of working.
Zusammenfassung
Traumatische Rupturen und Aneurysmen der thorakalen Aorta nach stumpfer Gewalteinwirkung wurden im letzten Jahrzehnt in zunehmendem Maße beobachtet. Nach dem Schrifttum und den eigenen Erfahrungen mit 23 Verletzten wurden bisher insgesamt 305 Patienten mit traumatischen Rupturen und Aneurysmen der thorakalen Aorta operativ behandelt. Die Operationsletalität von 87 Patienten, die im Frühstadium operiert wurden, lag bei 24%, die von 218 Patienten, die im Aneurysmastadium operiert wurden, bei 7 %.
Pathologie, Symptomatik, Diagnostik sowie Operationsverfahren und deren Hilfsmittel werden diskutiert. Die Methode der Wahl zur operativen Behandlung einer Aortenruptur oder eines postoperativen Aneurysmas ist unter Verwendung einer atrio-femoralen Blutumleitung und Heparinisierung des Patienten die Wiederherstellung der Gefäßkontinuität durch eine direkte Naht, eine End-zu-End-Anastomose der Aortenstümpfe oder durch Implantation einer Kunststoffprothese. Auch auf die für die Praxis sicherlich einfacheren Verfahren zur Operation der Aortenruptur bei Kombinationsverletzten wie den externen Shunt wird hingewiesen.
Im eigenen Krankengut kamen 6 Patienten mit einer akuten Ruptur und 17 mit einem posttraumatischen Aneurysma zur Aufnahme. Vier Aortenrupturen wurden 5 Std, 6, 8, und 18 Tage nach dem Unfall operiert; eine Patientin verstarb vor der Operation an einer Perforation des Rupturhämatoms in den Oesophagus, eine andere Patientin erlag 2 Std nach dem Unfall ihren schweren Begleitverletzungen. Von den 17 operierten posttraumatischen Aneurysmen verstarb ein Patient 5 Monate nach der Operation infolge einer Paraplegie. Die übrigen Patienten sind bis zu 12 Jahre nach der Operation beschwerdefrei und arbeitsfähig.
