Der Muskelschmerz mit schmerzhafter Muskelspannung, welcher auf Grund traumatischer Schädigung des Nerven an beliebiger Stelle seines Verlaufs zur Beobachtung kommt, kann durch Nervenblockade der über den schmerzenden Muskel hinziehenden suprafascialen Nerven behoben werden.

Eine suprafasciale Blockade der Punta maxima vonMackenzie kann bei chronischem viscerogenem Schmerz den Spontanschmerz und die Druckschmerzhaftigkeit auf eine, die Wirkung des Anästhetikums überdauernde Zeit aufheben. Die neurogene Myalgie wird in übereinstimmung mit den Versuchsergebnissen vonKellgren als ein besonderer Fall des tiefen übertragenen Schmerzes angenommen.

Es wird bewiesen, daß die Receptionsfelder des übertragenen tiefen Schmerzes und die Receptionsfelder der Hautoberfläche aus ein und demselben Nervenstamm hervorgehen. Diese Tatsache ermöglicht bestimmte hypothetische Deutungen des Wechselverhältnisses von Myalgie und Kausalgie sowie der wohltätigen Wirkung von Hautreiztherapien bei tief lokalisierten Schmerzen.

Der Axonreflex wird als peripheres Substrat der Ausbreitung der übertragenen Schmerzhaftigkeit vom Muskel bzw. vom Visceron auf das suprafasciale Schmerzfeld angenommen. In ähnlicher Weise hat diesLewis für die Ausbreitung der Schmerzhaftigkeit im Bereich des kutanen Receptionsfeldes getan. Außer diesem Mechanismus wird eine gleichsam synaptische, interaxonale Erregungsübertragung(Hering), innerhalb der afferente und efferente Fasern führenden Nervengeflechte erwogen. Derartige Nervengeflechte werden in den verschiedenen Schmerzreceptionsfeldern, insbesondere aber im periadventitiellen und adventitiellen Bindegewebe der Gefäße angetroffen.

Bestimmte Formen der Dauerschmerzen werden auf “reverberating circuits” in diesen peripheren Strukturen zurückgeführt. Es werden die besprochenen therapeutischen Erfolge einer suprafascialen Nervenblockade auf die Unterbrechung derartiger Circuli vitiosi bezogen.

Der Deutung der erhobenen Befunde wird die HypotheseWeddells und seiner Mitarbeiter bezüglich der Struktur des peripheren Substrats der Schmerzhaftigkeit zugrunde gelegt.

The musculary pain with painful muscle stretching which, caused by traumatic alterations of the nerves, may occur in any point of its course can be allayed with the blocking of the superfascial nerves running over the painful muscle.

A superfascial nerve-blocking applied inMackenzie's “puncta maxima” is able to abolish in cases of chronic viscerogenic pains the spontaneous aches as well as those after pressure for an amount of time, which is overgoing the action of anaesthesia. The neurogenic myaligia is supposed to be a special form of the pain transmissed by a deep pathway, according toKellgren experimental results.

It is proved, that recepting areas of the deeply transmissed pain and those of the skin surface are delivered by the same nervous truncus. This fact allows certain hypothetic interpretations concerning the relation between myalgia and causalgia as well as the wholesome effect of skin-irritating therapeutics in cases of pains with a deep localisation.

The axonic reflex is supposed to be the peripheric basis concerning the pathway of transmissed painfulness from the muscles respectively from the visceron to the suprafascial pain-area.Lewis was able to show in a similar way the extension of painfulness in the receptive cutaneous area. Besides this mechanism a synaptic like, interaxonal irritation-transmission (Hering) by afferent and efferent fibres in this nervous plexus is supposed. These nervous plexus are found in the different receptive pain-areas, especially in the peri-and adventitial connective tissue of blood vessels.

Certain forms of lasting pains are due to “reverberating circuits” in these peripheric structures. The therapeutic effects in a superfascial nerve-blocking concerning the interruption of these “circuli vitiosi” are discussed.

To the interpretation of the author's investigationsWeddell's and his coworkers hypothetic opinion of the peripheric basis structure of painfulness is submitted.

Les douleurs musculaires et une tension musculaire douloureuse, qui — dûes à des altérations traumatiques des nerfs, — peuvent se manifester à chaque point de leur parcours peuvent être supprimées à moyen des nerfs superfasciaux à l'enchaînement de la musculature douloureuse.

La blocade des nerfs superfasciaux appliquée au lieu des “puncta maxima” deMackenzie est en état d'abolir en cas de douleurs viscéraux chroniques les douleurs spontanées ainsi que celles à la suite d'une pression pendant un temps qui surmonte la durée de l'action de l'anesthésie. On suppose que la myalgie névrogène est une forme spéciale de la douleur à transmission profonde, ce qui est d'accord aux résultats expérimentaux deKellgren.

On prouve que les régions récepteures des douleurs à transmission profondes et celles de la surface de la peau appartiennent au même tronc névral. Cette constatation donne lieu à certaines interprétations hypothétiques à l'égard des rapports entre la myalgie et la causalgie ainsi qu'à l'effet avantageux d'une irritation thérapeutique de la peau au courant de douleurs à localisation profonde.

Le reflet d'axone est supposé d'être la base périphérique de la transmission de la douleur entre les muscles respectivement entre les organes viscéraux et la région superfasciale de la douleur. À l'aide de résultats analoguesLewis était en état de démontrer l'étendue de la douleur dans les régions cutanées récéptives. Outre ces méchanismes une transmission quasi synaptique interaxonale d'irritation (Hering) par voie des fibres afférentes et efférentes du tronc névral est supposée. Ces plexus nerveux ce trouvent dans les différentes régions récepteures de la douleur, spécialement dans le tissu conjonctif périet adventiciel des voies sanguines.

Si prova che le regioni ricettive del dolore a trasmissione profonda e quelle della superficie cutanea appartengono al medesimo tronco nervale. Questa costatazione dà luogo a certi interpretazioni ipotetici in riguardo ai rapporti entro la mialgia e la causalgia anche all'effetto vantaggioso di una irritazione terapeutica de la cute in caso di dolori a localizzazione profonda.

Si crede che il riflesso assono è la base periferica della trasmissione del dolore fra i muscoli, rispettivamente fra i organi viscerali della regione superfasciale del dolore. In base a risultati similiLewis are capace di dimostrare l'estensione del dolore nelle regioni cutanee ricettive. Oltre questi meccanismi una trasmissione quasi sinaptica interassonea d'irritazione (Hering) per via di fibre afferenti del tronco nervale è supposta. Questi complessi nervosi si trovano nelle differenti regioni ricettive del dolore, specialmente nel tessuto connettivo per e adventiziale dei vasi sanguinei.

Certe forme del dolore di durata si derivano dei “reverberating circuits” nelle strutture periferiche. L'effetto terapeutico del blocco dei nervi superfasciali in riguardo all'interruzione di questi “circuli vitiosi” è discusso.
