Zusammenfassung

Nach an sich belanglosen Verletzungen der Gliedmaßen können neben Schmerzen und vegetativen (vorwiegend vasomotorischen) Störungen funktionelle Lähmungen und Sensibilitätsausfälle zustande kommen. Diese Symptome werden anscheinend von der Verletzungs-bzw. Erkrankungsstelle (Focus) durch Vermittlung des protopathischen Systems ausgelöst und aufrecht erhalten. Sie beruhen wahrscheinlich sowohl auf spinalen reflektorischen Hemmungen als auch auf einer trophischen Störung, die zu einer Depolarisation sensibler und motorischer Fasern führt. Die Umspritzung der Verletzung mit einem Lokalanästheticum, die Grenzstrangblockade und die Lumbalanästhesie stellen in der Regel die sensiblen und motorischen Funktionen vorübergehend wieder her.

Summary

After even harmless injuries of the extremities there may arise functional paralysis and local loss of sensibility besides pain and vegetative (chiefly vasomotoric) reactions. These symptoms are induced and sustained by the injured spot (focus). The action of pain fibers and of the vegetative system probably lead to reflex inhibitions in the spinal cord. There is also evidence for a trophic disorder, which results in a depolarisation of sensory and motor fibers. These sensory and motor disturbances generally disappear, at least for some time, after injection of local anesthetics around the injured spot or after sympathetic block.

Résumé

Il est possible qu'aprés des blessures des extrémités, aussi insignifiantes, des paralyses temporaires et des troubles de la sensibilité se réalisent hors de douleurs et de troubles végétatifs (surtout vasomoteurs). Ces symptomes sont probablement déclenchés et maintenus à l'endroit de la blessure, par la médiation du système protopathique. Ils dépendent probablement non seulement des inhibitions spinales réflectogènes mais aussi d'un trouble trophique qui mène à une dépolarisation des fibres sensitives et motrices. Le trouble sensitif et fonctionnel généralement disparaît momentanément, au moins pour quelque temps soit par l'anesthésie locale autour de la blessure, soit par le blocage de a chaîne sympathique, soit par l'anesthésie lombaire.
