Über die Rückenmarkschirurgie des Schmerzes

Zusammenfassung

Kritischer Überblick über die anatomischen Grundlagen der Rückenmarkschirurgie des Schmerzes. Es zeigt sich, daß unserere Kenntnisse lückenhaft sind. Es wird die Vermutung ausgesprochen, daß der Hinterstrang wesentlich an der Schmerzleitung beteiligt ist. Histologische Befunde stützen diese Ansicht. Die Bedeutung des medullären Schmerzengramms (Moment vonLivingston) für Indikation und Art des Eingriffes wird hervorgehoben. Es wird vermutet, daß medulläre Schmerzen nicht über den Tractus spinothalamicus geleitet werden. Die Chordotomie ist der noch am besten fundierte Eingriff. Die Bedeutung des Rückenmarkes als integrierendes Organ wird betont.

Summary

Critical review of the anatomical bases of the spinal cord surgery in relation to pain. It appears that our knowledge is incomplete. The presumption has been uttered that the posterior cord participates essentially in the conduction of pain. Histological findings favor this opinion. The significance of the medullary pain-engram (Livingston's moment) for indication and kind of intervention is emphasized. It is presumed that medullary pains are not conducted over the spinothalamic tract. Cordotomy is still the best founded intervention. The significance of the spinal cord as integrating organ is emphasized.

Résumé

Revue critique des bases anatomiques de la chirurgie de la moelle épinière concernant la douleur dont notre connaissance est incomplète. Il semble que la corde postérieure participe essentiellement à la conduction de la douleur. Les résultats histologiques sont en faveur de cette opinion. L'importance de l'engramme de douleur médullaire (moment deLivingston) pour l'indication et pour la manière d'intervenir est accentuée. On présume que les douleurs médullaires ne soient pas conduites par le tractus spinothalamique. La chordotomie est encore l'intervention fondée le mieux. L'importance de la moelle épinière comme l'organe intégrant est accentuée.
