Dans le fascicule précédent, après une introduction sur le matériel et les méthodes employés, on a donné une revue complète de la clinique et de l'anatomopathologie générale qu'on rencontre chez les femmes porteuses d'une hyperostose frontale interne.
Dans cette seconde partie l'auteur analyse les particularités neuropathologiques: les mensurations crânio-cérébrales y sont données en détail (poids et épaisseur des parois de la calotte, mesures de la capacité crânienne, du poids du cerveau, du coefficient de Reichardt). L'étude de l'histopathologie cérébrale met en évidence une prédominance significative chez ces femmes d'une angiopathie dyshorique et d'une angiopathie congophile dans le cerveau, tandis que celle de l'artériosclérose du polygone de Willis permet l'élaboration de plusieurs conclusions fondamentales.
Enfin la discussion de ce travail démontre l'existence indubitable d'un réel syndrome deMorgagni-Morel, qui est ici prouvée par une étude détaillée de l'association entre eux des différents symptômes constituant le syndrome. Ce chapitre permet encore de mettre en évidence que la probabilité de rencontrer un tableau plus riche en symptômes est d'autant plus grande qu'on a affaire à une hyperostose de plus forte intensité.
Le travail se termine par quelques considérations pathogéniques qui situent la place du syndrome deMorgagni-Morel dans son cadre endocrinien.
Zusammenfassung
In dem vorhergehenden Heft wurde nach einer kurzen Einführung über das angewandte Material und die Methoden, ein möglichst vollständiger Überblick über die klinischen und allgemeinen pathologischen Befunde gegeben, die man bei Frauen mit interner frontaler Hyperostose festgestellt hat.
In diesem zweiten Teil analysiert der Autor die spezifischen neuropathologischen Merkmale: die Schädel- und Hirnmasse werden ausführlich geschildert (Gewicht und Dicke der Schädeldecke, Hohlmaß des Schädels, Gewicht des Gehirns, Koeffizient von Reichardt). Die histopathologische Untersuchung des Gehirns zeigt eine statistisch gesicherte Anhäufung einer dyshorischen sowie einer congophilen Angiopathie der intracerebralen Gefäße. Bei der Untersuchung der arteriosklerotischen Veränderungen des Circulus Willisii ergeben sich mehrere wichtige Schlußfolgerungen.
Schließlich weist die Diskussion dieser Arbeit auf das sichere Vorkommen des Syndroms vonMorgagni-Morel hin. Die das Syndrom bestimmenden Merkmale werden in einer genauen statistisch berechneten Analyse untersucht. Außerdem erlaubt diese Untersuchung den Schluß, daß ein um so reichhaltigeres Bild erwartet werden kann, je ausgeprägter die frontale Hyperostose ist.
Die Arbeit endet mit einigen pathogenetischen Erwägungen, die die Stellung des Syndroms vonMorgagni-Morel in seinem endokrinischen Rahmen bestimmt.
Summary
In the preceding fasciculus, after an introduction on the material and the methods employed, a complete review has been given of the clinical aspects and of the general pathology met with in women having an internal frontal hyperostosis.
In this second part the author analyses the neuro-pathological particularities: the cranio-cerebral measurements are here given in detail (weight and thickness of the walls of the skull-cap, measurements of the cranial capacity, of the weight of the brain, of Reichardt's coefficient). The study of cerebral histopathology makes evident a significant predominance in these women of dyshoric and congophil angiopathies in the brain, whereas the study of the arteriosclerosis of the circle of Willis allows the elaboration of several fundamental conclusions.
Finally, the discussion of this work shows the indubitable existence of a realMorgagni-Morel syndrome, which is here proved by a detailed study of the association among them of the different symptoms which constitute the syndrome. This chapter moreover makes evident that the probability of encountering a picture rich in symptoms will vary according to the intensity of the hyperostosis dealth with.
The work is concluded by some pathogenic considerations which determine the place of theMorgagni-Morel syndrome in the endocrine frame.
