Komplikationen nach Stellatumblockaden

Zusammenfassung

Bei zwei Patientinnen entwickelten sich nach einer Novocainblockade des Ganglion stellatum motorische, sensible und trophische Störungen am gleichseitigen Arm. In beiden Fällen war aber die homolaterate Körperhälfte am Krankheitsgeschehen beteiligt. Es ist sehr wahrscheinlich, daß bei der Stellatumblockade der Plexus brachialis verletzt wurde. Allerdings könnte nur ein Teil der genannten Störungen auf eine Unterbrechung der Nervenfasern zurückgeführt werden. Die restlichen Störungen scheinen, da eine Rückenmarksschädigung so gut wie ausgeschlossen werden kann, reflektorischer Natur gewesen zu sein. In dieser Hinsicht müssen jedoch auch konstitutionelle Faktoren (Erblichkeit, degenerative Stigma bei den zwei Patientinnen) in Betracht gezogen werden.

Summary

Two women patients who had undergone an anesthetic infiltration of the ganglion stellatum showed motoric, sensitive and trophic disturbances not only of the corresponding arm but also on the whole homolateral side of the body. Probably the plexus brachialis had been damaged while blocking the stellatum. But only part of the disturbances can be explained by the interruption of nervous fibres. As to the other symptoms they seem to be of a reflectory nature, for there was no symptom of deterioration of the spinal cord to be found. However, the influence of constitutional factors (heredity and degenerative stigmate be observed on the 2 patients) must be taken into account.

Chez 2 femmes ayant subi une infiltration anesthésiante du ganglion stellaire, des troubles moteurs, sensitifs et trophiques intéressant non seulement le bras correspondant mais toute la moitié homolatérale du corps se développèrent. Il est très probable que le plexus brachial ait été lésé lors de la piqûre. Toutefois, seulement una partie des troubles en question pourrai être expliquée par une interruption des fibres nerveuses. Quant aux autres symptômes, la nature réflexe en est vraisemblable, étant donné qu'une détérioration de la moelle épinière peut être excluse. Sous ce rapport, l'influence des facteurs constitutionaux (hérédité, stigmes dégéneratifs chez les 2 femmes) doit égalment être prise en considération.

Da due donne che avevano subito una infiltrazione anestiozzante del ganglion stellare disturbi motori sensitive e trofici si sono sviluppati colpenti non soltanto il braccio corrispondente ma tutta la metà omolaterale del corpo. È molto probabile che il plesso brachiale sia stato leso dalla puntura. Comunque soltanto una parte dei disturbi in questione potrebbe essere spiegatea con una interruzione di fibre nervose. Quanto agli altri sintomi è probabile la loro natura riflettoria siccome una deteriorazione del midollo spinale può essere esclusa. Sotto questo rispetto l'influsso di fattori costituzionali (eredità, stigmata degenerativi nel caso delle due donne) deve essere ugualmente considerato.
