Zusammenfassung
In unseren Untersuchungen befaßten wir uns mit dem Pathomechanismus der nach beiderseitiger cervicaler Vagotomie entstehenden Lungeninfiltration bzw. Vaguspneumonie. Wir konnten entgegen den bisherigen Theorien feststellen, daß bei der Entstehung der Vaguspneumonie eine neurale Komponente die wichtigste Rolle spielt, d. h. daß die Lungeninfiltration durch eine nach dem Ausfall der Vagusafferenz im hinteren Hypothalamusgebiet auftretende sympathische Aktivierung, die über die sympathischen Fasern der Lunge verläuft und eine Permeabilitätssteigerung hervorruft, zustande kommt.
Auf Grund unserer Untersuchungen spielen bei der Entstehung der Vaguspneumonie hämodynamische Faktoren keine Rolle. Wir konnten weiters feststellen, daß sozusagen parallel mit der Aktivierung des sympathico-adrenalen Systems auch die Hypophyse auf bisher noch unbekannter Weise an der Entstehung der Infiltration teilnimmt.
Summary
Experiments have been carried out to clarify the pathomechanism of pulmonary infiltration, the so-called “vagus-pneumonia”, developing after bilateral cervical vagotomy. Contrary to current theories, a neural factor was found to play the decisive role in the genesis of vagus-pneumonia: The loss of vagus-afferentation results in sympathetic activation in the posterior hypothalamic area, which, by means of increasing permeability via the sympathetic fibres to the lungs, gives rise to the infiltration.
The experiments also show that no haemodynamic factor is involved in the genesis of vagus-pneumonia. It has further been found that the participation of the hypophysis in the development of infiltration would seem to run a course somewhat fairly, parallel with the activation of the sympathico-adrenal system. However, the details of this parallelism require further investigation.
Résumé
Dans nos expériences nous nous sommes occupés du pathomécanisme de l'infiltration pulmonaire consécutive à la section cervicale bilatérale des nerfs pneumogastriques (pneumonie vagale). En opposition avec les théories connues jusqu'à nos jours, nous avons constaté que dans son pathomécanisme la composante neuronale joue le plus important rôle en tant que l'activation sympathique-établie dans la région hypothalamique postérieure et causée par l'abolition de l'afférentation vagale-détermine l'infiltration pulmonaire par l'intermédiation des fibres sympathiques du poumon. Selon nos expériences le facteur hémodynamique ne joue aucun rôle dans la genèse de la pneumonie vagale. Nous avons constaté aussi que la contribution de l'hypophyse dans le développement de l'infiltration se déroule parallèlement avec l'activation du système sympatho-surrénal par un mécanisme encore inconnu dans ses détails.
