Summary
A short review is given on the most important features of epidemiology, etiology, clinical course and pathomorphology of malignant hyperthermia. Five fatal cases are examined histologically. The most important findings are as follows:

The age of the patients varied between 4 and 22 years. During the episode the body temperature always exceeded 42° C. Rigidity or cramps of the skeletal muscles were not observed in three cases. All cases showed considerable metabolic acidosis. Death occured between 140 minutes and 17 hours after the beginning of general anaesthesia.

Histological examination revealed pathological alterations in the striated muscles of all patients.

There were findings indicating a mild preexisting myopathy: greater variation of the fiber diameters than normal with atrophic fibers either scattered diffusely or arranged in groups. Rarification of the subsarcolemmal nuclei, tendencies to central migration of nuclei, socalled „Kernreihen“, enhancement of the interstitial connective tissue.

Furthermore each case showed a different pattern of acute degeneration obviously corresponding to the time of survival: interstital edema, cloudy vacuolar or granular degeneration of fibers, hydropic swelling or pycnotic alterations of nuclei. Various types of hypercontraction figures, multiple fragmented fibers and empty sarcolemma.

The acute degenerative changes could be found even in those cases showing short periods of survival. They were present in the skeletal and in the heart muscles as well. Hypercontraction bands were to be seen even in those cases lacking rigidity during the episode.

In cases of acute death following general anaesthesia the striated muscles should be extensively examined to elucidate also such cases showing little evidence of malignant hyperthermia.

Darstellung der wichtigsten epidemiologischen, ätiologischen, klinischen und pathomorphologischen Daten zur Narkosekomplikation „Maligne Hyperthermie“. Untersuchung und Beschreibung von fünf letal verlaufenen Fällen.

Das Lebensalter lag zwischen 4 und 22 Jahren. Die Körpertemperatur überschritt regelmäßig 42° C. In drei Fällen fand sich kein Hinweis für Muskelrigor. Regelmäßig entstand eine metabolische Acidose. Der Tod trat 140 min bis 17h nach Narkosebeginn ein.

In allen Fällen fanden sich pathologische Veränderungen an der quergestreiften Muskulatur.

Im Sinne einer milden, präexistenten Myopathie Befunde wie: Kaliberschwankungen der Skelettmuskelfasern mit disseminierten und gruppierten Faseratrophien, Kernrarifizierung, Zentralkerne, Kernreihen, perimysiale Bindegewebszunahme.

Im Sinne einer akuten, durch die Episode ausgelösten Myopathie Befunde wie: interstitielles Ödem, Faserschwellung, vakuolige und granuläre Faserdegeneration, Kernschwellungen und -pyknosen, Hyperkontraktionsbanden und unterschiedlicher, zeitabhängiger Zerfall der Fasern und der Kontraktionsbanden, Faserrupturen, „bunte“ Muskelnekrobiosen.

Die akuten Befunde waren auch bei kurzer Überlebenszeit vorhanden und regelmäßig auch in der Herzmuskulatur nachweisbar, die Hyperkontraktionsbanden auch bei fehlendem Hinweis auf Muskelrigidität während der Episode.

Bei forensischen Fällen wird die generelle Erhebung eines histomorphologischen Muskelstatus bei jedem letalen Narkosezwischenfall gefordert.
