Zusammenfassung
In 5 Gruppen zu je 9–12 Probanden mit leichtem Diabetes mellitus, der keine Insulintherapie erforderte, wurden nachstehende Belastungsproben vor und während der Anwendung des Phenyläthylbiguanids ausgeführt: 1. Intravenöse Belastung mit 8 E. Insulin; 2. Intravenöse Belastung mit 1.0 g Tolbutamid; 3. Perorale doppelte Belastung mit 50 g Glucose; 4. Intravenöse Glucosebelastung unter Berechnung des Glucoseassimilationskoeffizienten; 5. Intravenöse Glucosebelastung wie oben, ausgeführt 90 Minuten nach peroraler Einnahme von 50 g Glucose. Die Blutzuckerkurven verliefen nach der Insulin- bzw. Tolbutamidverabreichung während der neuerlichen Belastung auf einem niedrigeren Niveau, wobei aber der prozentuelle Glucoseabfall keine Änderung erfuhr. Die Abnahme des anorganischen Phosphats, des Aminostickstoffs und des Leucins im Plasma, die normalerweise nach der Verabreichung von Insulin bzw. Tolbutamid eintritt, war bei gleichzeitiger Anwendung von Phenyläthylbiguanid etwas stärker. Die mittlere Blutzuckerkurve verlief nach doppelter Glucosebelastung während der zweiten Belastung bedeutend tiefer, wobei die Unterschiede der mittleren Glucosewerte in der 30. und 60. Minute der Untersuchung statistisch signifikant waren. Der Glucoseassimilationskoeffizient, der sowohl nüchtern als auch 90 Minuten nach peroraler Einnahme von 50 g Glucose bestimmt wurde, erfuhr unter dem Einfluß von Phenyläthylbiguanid keine merkliche Änderung. Die Verabreichung von Glucose allein rief hingegen einen deutlichen Anstieg des Glucoseassimilationskoeffizienten hervor. Die deutliche Änderung im Verhalten der Blutzuckerkurve nach peroraler Einnahme von Glucose bei unverändertem Wert des Glucoseassimilationskoeffizienten läßt wohl eine Wirkung von therapeutischen Gaben dieses Mittels beim Menschen im Bereich: Dünndarm-Leber vermuten. Gleichzeitig wurde eine gewisse Stärkung des Insulineffektes auf die Phosphorylierung und die Aminosäurenverwertung bei Anwendung dieser Mittel festgestellt.
Summary
Patients with mild diabetes that did not require insulin treatment were divided into 5 groups, each of 9 to 12 persons and one of the following tests performed on each before and during the administration of phenethylbiguanide: 1. the intravenous insulin tolerance test (8 u.); 2. the intravenous tolbutamide tolerance test (1.0 g); 3. the oral two-dose glucose tolerance test (2 × 50 g); 4. the intravenous glucose tolerance test, the coefficient of its assimilation being calculated; 5. the analogous intravenous test performed 90 min. after taking 50 g glucose by mouth. The glycemic curves obtained in the insulin and tolbutamide tolerance tests ran at somewhat lower levels during therapy; however, the rate of fall of blood sugar remained unchanged when expressed as percentage decrease. The decrease in plasma inorganic phosphate, amino nitrogen and free leucine usually found after the administration of insulin and tolbutamide was somewhat more marked when phenethylbiguanide was being given. The mean glycemic curve was considerably lower during the oral two-dose glucose tolerance test carried out during the treatment, the differences between the mean blood glucose values at the 30th and 60th min of the test being statistically significant. The coefficient of glucose assimilation determined fasting and at 90 min after the oral glucose uptake of 50 g of glucose was not appreciably affected by the treatment with phenethylbiguanide, although the glucose administration as such distinctly increased the coefficient both before and during the treatment. The modification of oral glucose tolerance with unchanged assimilation coefficient might indicate that in the human, therapeutic doses of phenethylbiguanide exert their effect at the level of the small intestine and the liver. The effect of insulin on glucose phosphorylation and on the utilization of aminoacids also seemed to be enhanced by this drug.
Résumé
Cette étude porte sur cinq groupes, dont chacun compte 9 à 12 sujets, souffrant tous d'un diabète de type bénin et non insulino-dépendant. Avant et après l'administration de phénéthylbiguanide, notre évaluation a porté sur cinq épreuves: 1. La surcharge endoveineuse d'insuline (8 u.). 2. Le test endoveineux au tolbutamide (1.0 g); 3. la double surcharge perorale au glucose (2 × 50 g); 4. la surcharge au glucose par voie intraveineuse avec calcul du coefficient d'assimilation; 5. la répétition de la surcharge au glucose par voie intraveineuse 90 minutes après l'ingestion per orale de 50 g de glucose. Les profils glycémiques après insuline et tolbutamide se sont situés à un niveau légèrement plus bas après traitement au phénéthylbiguanide; pourtant, la pente de la dépression du glucose sanguin, exprimé en pour cent du taux initial, est restée la même. L'administration d'insuline et de tolbutamide a mené à la dépression habituelle des taux plasmatiques de phosphate inorganique, d'azote aminé et de leucine libre, et ceci de façon légèrement plus marquée après qu'avant le traitement. Le profil moyen de la glycémie durant la surcharge perorale double au glucose a été nettement plus bas durant le traitement, les différences entre les valeurs obtenues à la 30ème et 60ème minute étant statistiquement significatives. Les coefficients d'assimilation du glucose mesurés avant et 90 minutes après l'ingestion de 50 g de glucose n'ont pas été modifiés par le traitement à la phénéthylbiguanide, l'ingestion de glucose augmentant distinctement le coefficient dans les deux cas. Une modification de la courbe d'hyperglycémie provkquée par voie orale sans altération du coefficient d'assimilation du glucose pourrait indiquer que, chez l'homme, des doses thérapeutiques de phénéthylbiguanide peuvent exercer leur effet au niveau de l#intestin et du foie. Il semblerait également que cet agent augmente légèrement l'action de l'insuline sur la phosphorylation du glucose et sur l'utilisation des acides animés.
