Summary
The influence of different temperatures between 13° C and 45° C on coagulation factors in vitro was studied by measuring clotting time with the recalcification time, partial thromboplastin time (PTT), and thromboplastin time test. In all three tests the shortest clotting times were measured at a temperature of 40° C. The relation between temperature and clotting time was similar in fresh plasma and in plasma which had been stored at a temperature of −20° C before examination. However, in all tests stored plasma showed shorter coagulation times. Prolongation of coagulation time at 45° C is caused by irreversible reduction of coagulation activity in the plasma. At the same time thromboplastin- and PTT-reagent are impaired in their coagulation activity by a temperature of 45° C. In comparison to plasma obtained from healthy persons plasma from patients with hemophilia A or B or with v. Willebrand's disease reacted more sensitive to changes in temperature in the PTT test. The coagulation defect was definitely more pronounced at 27° and 17° C than at 37° C. It was not possible to differentiate these three coagulopathies with the PTT test at 27° and 17° C.
Zusammenfassung
In der vorliegenden Arbeit wird der Einfluß verschiedener Temperaturen zwischen 13° C und 45° C auf die plasmatische Blutgerinnung in vitro untersucht an Hand von Gerinnungszeitmessungen im Plasma-Rekalzifizierungszeit-, partiellen Thromboplastinzeit-(PTT) und Plasma-Thromboplastinzeit-Test. In allen drei Testen erreichen die Gerinnungszeiten in Abhängigkeit von der Temperatur übereinstimmend ein Minimum bei 40° C. Der Verlauf der Temperaturabhängigkeit der Gerinnungszeiten ist bei Plasmen, die vor ihrer Untersuchung bei -20° C gelagert wurden ähnlich wie bei frisch untersuchten. Die gelagerten Plasmen weisen jedoch in allen Testen kürzere Gerinnungszeiten auf. Die Verlängerung der Gerinnungszeiten bei 45° C wird durch eine irreversible Verminderung der Gerinnungsaktivität des Plasmas bewirkt. Thromboplastinlösung und PTT-Reagenz werden jedoch ebenfalls durch Hitzeeinwirkung von 45° C in ihrer Gerinnungswirksamkeit beeinträchtigt.
Im Vergleich zu den Plasmen von Gesunden reagieren jene von Patienten mit Hämophilie A, B oder v. Willebrandscher Krankheit im PTT-Test empfindlicher auf Temperaturänderungen: ihre Gerinnungsstörung tritt bei 27° C und 17° C deutlich stärker in Erscheinung als bei 37° C. Eine Unterscheidung dieser drei Koagulopathien ist jedoch mit dem PTT-Test durch Vergleichsmessungen bei 27° C und 17° C nicht möglich.
