Summary
A patient with “chronic cold-agglutinin disease” is described. Initially, the process was a benign monoclonal gammopathy. After an intervall of 6 years clinical, cytomorphological and immunochemical studies showed progress towards malignancy. In the bone marrow intranuclear lipid-like inclusions appeared in the plasma cells.
Zusammenfassung
Bei einem Patienten mit chronischer Kälteagglutinin-Krankheit wurde die Entwicklung von einer benignen monoklonalen Gammopathie zu einer malignen Proteinämie verfolgt. Nach einem relativ gutartigen Verlauf von 6 Jahren zeigte sich ein Anstieg von IgM und die Ausscheidung von Paraproteinen im Urin. Gleichzeitig kam es zur Vermehrung der Lymphozyten im peripheren Blut und zur Vermehrung von lymphoiden Zellen und Plasmazellen im Knochenmark. Etwa 10% der letzteren zeigten große, Sudan B-positive Vakuolen, die auch elektronenmikroskopisch dargestellt werden.
