Zusammenfassung
Der Myeloblast ist nur wenig größer als der Lymphozyt und hat dieselbe Kern/ Plasma-Relation von ca. 0,7, aber unterscheidet sich von ihm durch seine Morphologie, seine häufigen Mitosen und sein kinetisches Verhalten. Durch seine Transformation zum Promyelozyten ist er als granulozytopoetische Vorstufe ausgewiesen. Wie der Lymphozyt ist der Myeloblast in hohem Grad, wenn auch mit geringerer Geschwindigkeit, zur Fortbewegung fähig. Dadurch tauscht er sich wie der Lymphozyt zwischen Knochenmark und Blut aus. Von der granulozytopoetischen Reihe sind damit nur der Promyelozyt und der Myelozyt mit der Kern/Plasma-Relation von 0,3 im Knochenmark sessil.
Der Monoblast hat dieselbe Kern/Plasma-Relation und fast dieselben morphologischen Eigenschaften wie der Myeloblast und ist etwa doppelt so groß. Er wird etwas seltener in Fortbewegung angetroffen. Außer zur Reifung zum Promonozyten, später Monozyten und Histiozyten steht dem Monoblasten noch ein anderer Entwicklungsweg offen: er kann mit demselben Kern/Plasma-Verhältnis (0,7) zu einer wiederum doppelt so großen Zelle anwachsen, wobei viele große, runde Granula im Zytoplasma entstehen: Gewebehistiozyten oder Gewebemastzelle. Demnach wächst eine sich häufig teilende, basophile Zelle mit netziger Kernstruktur und Nukleolen, der Hämozytoblast, von einem Kerndurchmesser 6,5 auf 13,0 μm an, bekommt das Ferment α-NA-Esterase und zuletzt große Lysosomen. Ist diese Zelle noch klein (Kernklasse 1/4 bis 1/2), kann sie in die granulozytopoetische Reihe stimuliert werden, wenn sie größer ist (K1/2 bis K1), in die monozytopoetische. In der Kerngröße K1 bis K2 kann die Stammzelle durch Erythropoetin in die Erythropoese stimuliert werden. Erfolgt keine Stimulierung, degeneriert sie zuletzt als granulierte K2-Zelle. Nach unseren Beobachtungen hat die Lymphopoese beim Erwachsenen keine Beziehung zu der granulozyto-monopoetisch determinierten Stammzelle, weswegen ein dualistisches hämatopoetisches System postuliert wird.
Bei unreifzelligen Leukämien streuen die Zell- und Kerngrößen breiter und kinetisch aktive Stammzellen sind seltener, jedoch können sich die leukämischen Promyelozyten im Gegensatz zu den normalen fortbewegen. Bei den leukämischen Myeloblasten kommen weniger Mitosen vor als bei den normalen. Bei den leukämischen Promyelozyten und Monoblasten ist die Mitosehäufigkeit nicht reduziert.
Summary
The myeloblast is nearly of the same size as the lymphozyte and has the same nucleus cytoplasma ratio of 0.7 but differs from it in its morphology, its high frequency of multiplication, and by its kinetic behaviour. By phase contrast observation the transformation of the myeloblast into the promyelocyte has been seen several times. The myeloblast like the lymphocyte is able to move although at a much slower speed. By locomotion the myeloblast, and likewise the lymphocyte can move between blood and bone marrow. Of the granulocytopoietic series only the promyelocyte and the myelocyte with their nuclear cytoplasm relation of 0.3 are found in the bone marrow. formation of the myeloblast into the promyelocyte has been seen several times. The monoblast has the same nucleus cytoplasm ratio and nearly the same morphology as the myeloblast but is about twice its size, and moves less often. Apart from its transformation into promonocyte, monocyte and histiocyte, the monoblast may evalue in another way: it grows into a cell of doubled size with the same nucleus cytoplasm ratio 0.7 in whose cytoplasm are seen a lot of big granules. The cell may now be characterized as a tissue macrophage or mastcell. In consequence a frequently multiplying basophilic cell with a coarse nuclear structure and nucleoli, grows from a diameter of 6.5 to one of 13.0 μm without marked morphologic change apart from becoming α-NA-Esterase positive and acquiring big lysosomes. When this haemocytoblast is small (K1/4 to K1/2) it can be triggered into the granulocytopoietic series, when it is bigger (K1/2 to K1) into the monocytopoietic series, or in the size K1 to K2 by erythropoietin into the erythropoietic series. If no triggering takes place, the cell degenerates after having granulated. The lymphopoietic system is not connected to this system of stem cells committed to the granulocytopoietic, the monocytopoietic and the erythropoietic series. Therefore we postulate a dual haematopoietic system.
In acute leukemias the spread of size of cells and nuclei is wider and kinetically active stem cells are rarer. Only leukemic promyelocytes are in locomotion vividly, in contrast to the normal ones. In leukemic myeloblasts there are fewer mitoses than in normal ones, but in leukemic promyelocytes and monoblasts the mitotic frequency is not reduced.
