I.
Das infiltrierende (invasive) Wachstum kann beimCarcinom in zwei Formen vor sich gehen: Einmal werden die ortsstÄndigen Faserstrukturen zur Seite gedrÄngt und mit in die das Epithelfeld umgebende Basalmembran einbezogen (komprimierend-infiltrierendes Wachstum); das andere Mal wachsen die Epithelzellen ohne Bildung einer deutlichen Basalmembran zwischen die kollagenen und Reticulinfasern vor und schlie\en sie zwischen sich ein (interstitiell-infiltrierendes Wachstum); beide Formen des infiltrierenden Wachstums können ineinander übergehen. Das Erhaltenbleiben von Faserstrukturen zwischen Epithelzellen, das sich übrigens auch in Keratoakanthomen nachweisen lÄ\t, spricht gegen eine enzymatisch-lytische Wirksamkeit der Epithelzellen. Als wesentlich für das Vordringen des Krebses wird für beide Wachstumsarten der durch die hÄufigere Zellteilung hervorgerufene innere Druck im Zellverband angesehen.

Im sog.Carcinoma in situ der Portio finden sich im Stadium der Frühinvasion des Stromas dieselben Wachstumsformen wie beim ausgebildeten Carcinom (I): Es wÄchst teils komprimierend unter Erhaltung der Basalmembran, teils interstitiell unter Verlust der Basalmembran. Ihre Zerstörung dürfte hier wie dort auf entzündlich-lymphocytÄre Infiltrate zurückgehen.

The invasive growth may occur in two forms: 1. The local fibrillar structures are pushed aside and incorporated into the basement membrane surrounding the epithelial pegs (compressing-invasive growth); 2. the epithelial cells grow, without forming a distinct basement membrane, amidst the collagenous and reticulin fibers which finally are included between the epithelial cells (interstitial invasive growth); both forms of invasive growth may change into one another. The preservation of fibrous structures between the epithelial cells, also easily demonstrated in keratoacanthomas, is strong evidence against an enzymatic (lytic) activity of the epithelial cells on fibers and basement membranes. The increased internal pressure within the tumor caused by the frequent cell multiplication seems to be the essential force behind the invasive advance of the tumor cells.

II.
The same forms of invasive growth can be observed in the so-called carcinoma in situ of the uterine cervix during the “early stromal invasion”, as in the established carcinoma. The carcinoma in situ in part invades with compression but with preservation of the basement membrane, and in part by interstitial invasive growth with loss of the basement membrane. The disappearance of the basement membrane occurs at the site of lymphocytic infiltrations.
