Zusammenfassung
Die Hypophysenvorderlappen von 19 Diabetikern werden cytologisch quantitativ ausgezählt und gleichzeitig die quantitative Zusammensetzung der Langerhansschen Inseln kontrolliert. Die Fälle verteilen sich auf folgende Gruppen:

Labiler „juveniler“ magerer Diabetes 6 Fälle

2.
Stabiler, milder l“Diabète gras” 9 Fälle
-
3.
Perakuter juveniler Diabetes 2 Fälle
-
4.
Insulin-resistenter Diabetes 2 Fälle
Die Fälle der Gruppe l weisen einen hochgradigen B-Zellmangel der Langerhansschen Inseln und statistisch signifikant erhöhte Acidophilenanteile in den Hypophysenvorderlappen auf (durchschnittlich 59,4%), gegenüber den aufRasmussen basierenden Normalwerten. Gruppe 2 weist demgegenüber Acidophilenzahlen auf, die den Normalwerten vonRasmussen undSommers entsprechen. Bei den perakuten Fällen in Gruppe 3 unterscheiden sich nur die Langerhansschen Inseln qualitativ von den übrigen Gruppen. Dabei ist das Bestehen einer lymphocytären Infiltration der Inseln in einem Fall hervorzuheben. Die zwei Fälle mit Insulin-resistentem Diabetes fallen ebenfalls durch einen hohen Acidophilen-Prozentsatz im Hypophysenvorderlappen auf, hingegen zeigen die Langerhansschen Inseln mäßig reduzierte bis normale B-Zellwerte mit reichlich Granula.
Morphologisch ist mit dieser Zählmethode die pathogenetische Differenzierung des Diabetes mellitus möglich, indem die Störung bei Gruppe 1 auf der Achse Hypophyse/Langerhanssche Inseln zu suchen ist, während sie sich in den Fällen der Gruppe 2 auf der Achse Langerhanssche Inseln/Insulin verbrauchende Zelle findet.
Summary
The anterior pituitary and the islets of Langerhans of 19 patients with diabetes mellitus were subjected to a quantitative cytological investigation. These cases belonged to the following groups:

1.
unstable, juvenile diabetes 6 cases
-
2.
stable, mild diabetes (“Diabète gras”) 9 cases
-
3.
acute diabetes (lethal within 15 days) 2 cases
-
4.
insulin resistant diabetes 2 eases
The cases of group 1, which showed a reduced number of B-cells in the islets of Langerhans, had increased numbers of acidophils (59.4% on the average) in the anterior pituitary, as compared with the three other groups. This difference was even more pronounced when age and sex were considered. The number of acidophils in the pituitaries (40.3% on the average) of group 2, however, compared well with the normal values given byRasmussen andSommers.
In the cases with the rapidly fatal course (group 3), the various cell types differed greatly in their morphologic appearance, but were not abnormally distributed.
A great number of acidophils was observed in the pituitaries of the insulin resistant diabetics, whereas the B-cells of the pancreatic islets were normal or reduced in number but rich in granules.
This quantitative method allows one to distinguish the pathogenesis of the unstable, juvenile diabetes from that of the stable, mild forms of the disease. The first reveals a disturbance of the pituitary — pancreas axis, and the latter a disturbed pancreas — (insulin-consuming) cell axis.
