Zusammenfassung
Das dystelektatische Emphysem ist ein herdförmiges, sekundäres, zumeist bronchiologen bedingtes Emphysem, wobei sich das restliche Lungengewebe in einem der Atelektase nahen Zustand befindet.
Bei 300 Autopsien, bei denen die Lungen eingehend studiert wurden, wurde 86mal dystelektatisches Emphysem festgestellt, am häufigsten (44mal) als Ursache eines Cor pulmonale. Es wurde jedoch auch bei anderen Störungen des Lungen-kreislaufs und sogar in sonst völlig normalen Lungen bei normalen Kreislaufver-hältnissen gefunden. Eine Analyse der Befunde führt zu dem Schluß, daß das dystelektatische Emphysem erst dann pathologisch wird, wenn die aktive Steigerung der Inspiration und der Blutstrom in den Venulae versagen. Die Venulosklerose bei dystelektatischem Emphysem entsteht wahrscheinlich meistens infolge unrichtiger Verteilung der Lungengase und infolge des so ausgelösten reflektorischen Dauerspasmus der Gefäße. Die Venulosklerose führt zur Lungen-induration und zur anatomischen „Fixierung“ der pulmonalen Hypertonie sowie des gesamten pathologischen Zustands.
Summary
The dystelectatic emphysema is a focal, secondary, mostly bronchiologenic emphysema, in which the remaining lung tissue is found in a condition approaching atelectasis.
In 300 autopsies, in which the lungs were carefully studied in detail, dystelectatic emphysema was detected 86 times, most commonly (44 times) as the cause of cor pulmonale. It was also found, however, in other disturbances of the pulmonary circulation, and even in completely normal lungs with normal circulatory conditions. An analysis of the results led to the conclusion that the dystelectatic emphysema first becomes pathologic when the active increase in the inspiration and the blood-flow through the venules fail. Probably the sclerosis of the venules in dystelectatic emphysema develops mostly because of the improper distribution of the pulmonary gases and because of the lasting vascular spasm resulting reflexly therefrom. The sclerosis of the venules leads to an induration of the lungs and to the anatomic „fixation“ of the pulmonary hypertension as well as of the whole pathologic state.
