Zusammenfassung

An einem 26jährigen Manne wurde wegen bilateraler chronischer Pyelonephritis eine Nierentransplantation vorgenommen; Spender war der lebende, gesunde Bruder. Die transplantierte Niere funktionierte 6 Wochen gut, dann ließ die Funktion nach und nach 79 Tagen trat der Tod ein. In der transplantierten Niere war das Parenchym im großen und ganzen erhalten, doch zeigten die Tubuli ein an ischämische Nephrose erinnerndes Bild mit regressiven und regenerativen Veränderungen. Als am wesentlichsten werden die in allen mittelgroßen und großen intrarenalen Arterien und — weniger ausgesprochen — in den Venen festgestellten obliterierenden Veränderungen betrachtet, welche durch fibröse Verdickung der Intima starke Verengung oder gar Verschluß des Lumens zu infarktartigen Nekrosen führen. Als Ursache dieser Veränderungen kommen Hypertonie und immunologische Faktoren in Betracht, bei den Venen auch eine Thrombose.

Summary

A renal transplantation was undertaken in a 26 year old man because of bilateral chronic pyelonephritis. The healthy brother served as donor. For six weeks the transplanted kidney functioned well, but thereafter its function diminished, and after 79 days death occurred. The parenchyma in the transplanted kidney was largely preserved, although the tubules revealed regressive and regenerative changes resembling those of ischemic nephrosis. The obliterating vascular changes, however, were considered to be the most important, involving all middle and large-sized intrarenal arteries, and to a lesser degree the veins. By means of fibrous thickening of the intima and intense narrowing of the vascular lu mens, or even occlusion of them, infarct-like necroses were produced. Hypertension and immunological factors are considered as causes of these changes; in the veins thrombosis may also be a cause.
