Zusammenfassung
Der typischen Symptomatologie des Duane-Syndroms mit Abduktionslähmung, Verengerung der Lidspalte und Retraktion des Bulbus bei Adduktion wird ein atypisches Syndrom mit Adduktionslähmung, intakter Abduktion und Lidspaltenverengerung, sowie Retraktion bei Intention zur Adduktion gegenübergestellt. Mit Hilfe der Elektromyographie der Augenmuskeln (gleichzeitige Ableitung von Rectus externus und Rectus internus am befallenen Auge) können beide Formen des Duane-Syndroms als Folgen einer paradoxen Innervation zwischen Rectus externus und Rectus internus gedeutet werden: beim typischen Duane-Syndrom (I) hat der Rectus externus sein Innervationsmaximum bei der Intention zur Adduktion; beim atypischen Duane-Syndrom (II) zeigt der Rectus externus zwei Innervationsmaxima, eines bei der Adduktion und ein anderes bei Abduktion. Abduktionslähmung beim Duane I und Adduktionslähmung beim Duane II, aber auch die Retraktion des Bulbus in der Adduktion bei beiden Formen lassen sich auf Grund dieser Innervationsphänomene überraschend einfach erklären. Die Duane-Syndrome werden in den noch umfassenderen Rahmen kongenitaler zentraler Innervationsstörungen (Papst-Esslen) hineingestellt, denen ein paradoxer Synergismus zwischen Augenmuskeln, die von verschiedenen Nerven versorgt werden, zugrunde liegt. Topisch-diagnostische und differentialdiagnostische Betrachtungen. Therapeutische Hinweise für eventuelle operative Korrektur unter Berücksichtigung der elektromyographischen Befunde.
Summary
There are two forms of the Duane Syndrome which from the clinical standpoint can be differentiated: First Duane I with palsy of abduction, narrowing of the lid fissure and retraction of the eye in adduction; second Duane II with palsy of adduction with normal abduction and narrowing of the lid fissure and retraction in adduction. By means of electromyography it can be demonstrated that these “palsies” are due to a paradoxical innervation between external and internal rectus: in the typical Duane Syndrome (I) the external rectus has its maximum of innervation in adduction and its minimum of innervation on intention to abduction. In the atypical Duane Syndrome (II) the external rectus manifests two maxima of innervation, one on adduction and the other on abduction. Abduction palsy in the Duane I and adduction palsy in the Duane II, but also the retraction of the eye in adduction can be explained by these innervation phenomenoma very easily. The Duane Syndrome is to be regarded as a special form of congenital central innervation disorders (Papst-Esslen) where a pathological synergistic innervation between muscles which work normaly mainly as antagonists occurs.
