Summary
The endocrine function of the ovaries after the menopause has been studied. We were interested in the correlation of the morphological and enzymehistochemical dates to the morphology of the endometrium and the estrogen level in the urine before and after oophorectomy.
The ovaries were obtained by hysterectomy and adnectomy: Carcinoma of the endometrium was found in nine patients and in one case there was an adenomatous hyperplasia of the endometrium. Light and electronmicroscopical and also enzymehistochemical studies were performed on the ovaries. Furthermore the endometrium, cervix and both tubes histologically were examined. In all patients urinary estrogen level was determined before and after surgery.
In most of the postmenopausal ovaries a cortical stromal hyperplasia was found by light microscopical examination. Moreover in many ovaries hilus cells were seen. In the hyperplastical region of the cortex polygonal cytoplasm-rich stroma cells were noticed. They formed small clusters or were scattered distributed in the cortex area. The cytoplasm-rich stroma cells and the hilus cells show typical ultrastructural characteristics of steroid cells: smooth endoplasmic reticulum, tubular-vesicular mitochondria and heterogeneous lipofuscin granula.
Enzymeactivities of the glycolyse, high enzymeactivites of the pentose-phosphate-shunt, and some activities of the 3β- and theΔ5-3β-hydroxysteroid-dehydrogenase were obtained by enzymehistochemical examination of the steroid cells. According to the morphological and enzymehistochemical characteristics were the polygonal cytoplasm-rich stroma cells of the ovarial cortex and the hilus cells part of the steroid synthesizing cell system. Parameters of their function were the decreasing estrogen level before and after oophorectomy and the effect of estrogen on the endometrium.
Zusammenfassung
Um weitere Einblicke in die endokrine Funktion der Ovarien nach der Menopause zu erhalten, interessiert die Korrelation ihrer morphologischen und enzymhistochemischen Befunde zur Morphologie des Endometriums und zu prä- und postoperativ gemessenen Oestrogenwerten im Harn.
Die Ovarien wurden bei Totalexstirpation von Uterus und Adnexen gewonnen. Die Operation erfolgte bei neun Patientinnen wegen eines histologisch gesicherten Endometriumcarcinoms, in einem Fall wegen einer adenomatösen Hyperplasie des Endometriums.
Es erfolgten licht- und elektronenmikroskopische sowie enzymhistochemische Untersuchungen der Ovarien. Zur histologischen Untersuchung gelangten außerdem der Tumor des Endometriums, das tumorfreie Restendometrium, die Cervix sowie beide Tuben. Ergänzend wurden bei den Patientinnen prä- und postoperativ Oestrogene im Harn bestimmt.
Die lichtmikroskopische Untersuchung ergab in den meisten Postmenopauseovarien eine Stromahyperplasie der Rindenzone. Darüber hinaus konnten in zahlreichen Ovarien Hiluszellen gefunden werden. Außerdem fielen in der hyperplastischen Rindenzone polygonale cytoplasmareiche Stromazellen auf. Sie bilden kleine Gruppen oder liegen diffus verteilt in der Rindenzone. Die großleibigen Stromazellen sowie die Hiluszellen weisen typische ultrastrukturelle Merkmale der Steroidzellen auf: Glattes endoplasmatisches Reticulum, tubulovesiculäre Mitochondrien sowie heterogene Lipofuscingranula. Durch parallel laufende enzymhistochemische Untersuchungen wurden in den Steroidzellen Aktivitäten der Dehydrogenasen, der Glykolyse, hohe Aktivitäten von NADPH-liefernden Enzymen sowie nachweisbaren Aktivitäten der 3β- und derΔ5-3β-Hydroxysteroid-Dehydrogenase festgestellt. Aufgrund der morphologischen und enzymhistochemischen Eigenschaften gehören polygonale Stromazellen der Ovarrinde und Hiluszellen dem steroidbildenden Zellsystem an.
Als Parameter ihrer Funktion kann der Oestrogenabfall aus prä- und postoperativ bestimmten Oestrogenwerten sowie der Oestrogeneffekt am Endometrium der 10 untersuchten Fälle gelten. Bei 9 Patientinnen konnten neben einem Carcinom des Endometriums verschiedene Proliferationsgrade, wie glandulär-cystische und/oder adenomatöse Hyperplasien, im tumorfreien Restendometrium beobachtet werden. Bei einer Patientin handelte es sich um eine adenomatöse Hyperplasie des Endometriums ohne gleichzeitiges Vorliegen eines Carcinoms.
