Summary
Prior to hysterectomia and x-ray therapy, 23 patients with adenocarcinoma, one with leiomyosarcoma and one with atypical endometrial glands were treated with gestagens for two months. The following observations were made after hormonal treatment: In the last case endometrial atypia could not be further observed. No carcinomatous tissue was found in 5 cases of adenocarcinoma, this being probably due to abrasio before treatment. Changes toward a secretory endometrium were found in 6 highly differentiated tumours.
In 5 cases the glucose metabolism of carcinomatous tissue was studied in vitro before and after gestagen treatment of the patient. In one highly differentiated adenocarcinoma the histological and metabolic data were unchanged. On the other hand, in four lower differentiated tumours the rate of the in vitro metabolism of glucose was stimulated to a great extent by the hormonal treatment.
In 5 cases the in vitro influence of progesterone on the metabolism of glucose was investigated. In contrast to the changes observed in the cancerous tissue after hormonal treatment of the patient, the in vitro application of progesterone caused inhibition of the energy metabolism.
The correlations between the histological and the biological observations and their significance for carcinomatous growth are discussed. Further investigations on the effect of gestagens on glycogen synthesis are suggested.
Zusammenfassung
Bei einem ausgewählten Material von 23 Adenocarcinomen, einem Leiomyosarkom und einem Fall von Drüsenzellatypien wurde vor konventioneller Therapie—Hysterektomie und radiologische Behandlung—eine 2monatige gestagene Therapie durchgeführt. In 5 Fällen von Adenocarcinom wurde im Operationspräparat kein carcinomatöses Zellmaterial vorgefunden, was auf eine primäre Reinausschabung zurückgeführt wird. Im Fall der Drüsenzellatypien waren nach der Hormonbehandlung keine Atypien mehr nachweisbar.
6 Fälle von höher differenzierten Adenocarcinomen wiesen im Operationspräparat Zeichen sekretorischer Zellumwandlung auf. In 5 Fällen wurde der in vitro-Glucosestoffwechsel vor und nach gestagener Behandlung des Patienten untersucht. In einem Fall eines höher differenzierten Adenocarcinoms konnten weder histologische noch stoffwechselmäßige Veränderungen durch die Behandlung festgestellt werden. Dagegen wurden in 4 Fällen, die zum größten Teil aus niedrig differenzierten Carcinomen bestanden, der Glucosestoffwechsel des Carcinomgewebes nach Hormonbehandlung des Patienten stark erhöht.
5 Fälle wurden in Bezug auf die in vitro-Einwirkung von Progesteron auf den Glucosestoffwechsel des Carcinomgewebes studiert. Im Gegensatz zum Effekt der gestagenen Behandlung des Patienten hemmte aber in vitro-appliziertes Progesteron den Glucosestoffwechsel.
Die Bedeutung der histologischen und biochemischen Beobachtungen in Bezug auf das carcinomatöse Zellwachstum und deren eventuell gemeinsam zu Grunde liegender Vorgang in der Zelle werden diskutiert. Weitere Untersuchungen, vor allem der Glykogensynthese unter hormoneilen Einfluß, werden für notwendig erachtet.
