Summary
The processes of metaplasia and basal hyperplasia induce epithelial transormation. Carcinomas in situ, most dysplasias and the so-called abnormal epithelium are final forms of epithelial transformation. All types of pathological epithelium are situated in limited fields. Thus they are clearly defined from the normal squamous and columnar epithelium and also among each other. There is nothing to suppose an active spread of one epithelial type encroaching upon the other. The last cervical gland is a border marking the point where the squamocolumnar junction was originally situated. Squamous epithelium lying above the glandular field has thus developed secondarely due to squamous metaplasia. The only exception is the epithelization of a rare true erosion in the columnar epithelium by ascending healing. Epithelial lesions peripheral to the glandular field, however, probably developed in the original squamous epithelium by means of basal hyperplasia. Provided there is no active spread of a pathological epithelium the latter will not, as a rule, extend beyond the last cervical gland. The revision of the examined material has shown that only in 10 out of 295 epithelial foci a completely uniform epithelium extended beyond the last gland. But even these exceptions can be explained without suggesting an active spread of the atypical epithelium. Also the different localization of various epithelial types and their symmetrical distribution, as shown statistically, both prove the fact that epithelia developed on the spot of their location without any further spread. Thus the clearly defined dysplasias and carcinomas in situ do not constitute sequential stages of the same developmental process. The localization of pathological epithelia of various differentiation depends on the type of matrix on which the atypia develops: The higher the differentiation of an atypical squamous epithelium the more likely it developed by transformation of squamous epithelium-either of an original squamous epithelium or a squamous epithelium having at first developed by means of squamous metaplasia in the glandular field. The lower the differentiation of an atypical epithelium the more probable its genesis will by directly due to an atypical squamous metaplasia.
Zusammenfassung
Metaplasie und basale Hyperplasie sind Vorgänge, die einen Epithelumbau einleiten. Carcinomata in situ, die meisten Dysplasien und abnormes Epithel sind fertig umgebaute Epithelformen. Alle pathologischen Epithelformen liegen in begrenzten Feldern. Sie sind daher zum normalen Plattenepithel und zum Cylinderepithel, aber auch untereinander, scharf abgegrenzt. Für die Annahme einer aktiven Ausbreitung eines Epitheltyps auf Kosten eines anderen gibt es keinen Anhaltspunkt. Die letzte Cervixdrüse ist ein Grenzpunkt, der anzeigt, wo ursprünglich originäres Plattenepithel auf Cylinderepithel gestoßen ist. Plattenepithel, das über dem Drüsenfeld liegt, ist demnach sekundär durch Plattenepithelmetaplasie entstanden. Eine Ausnahme ist nur die Heilung der seltenen echten Erosionen im Cylinderepithel durch aufsteigende Überhäutung. Epithelveränderungen außerhalb des Drüsenfeldes müssen hingegen, wahrscheinlich auf dem Wege der sogenannten basalen Hyperplasie, im originären Plattenepithel entstanden sein. Falls es keine aktive Ausbreitung eines pathologischen Epithels gibt, dürften solche Epithelien in der Regel auch nicht über die letzte Drüse hinwegreichen. Die nochmalige Durchsicht des untersuchten Materials hat gezeigt, daß nur in 10 von 295 Epithelherden ein völlig gleichförmiges Epithel über die letzte Drüse hinwegzieht. Aber auch diese Ausnahmen können erklärt werden, ohne daß eine aktive Ausbreitung des atypischen Epithels angenommen werden müßte. Auch die Unterschiede in der Lokalisation der verschiedenen Epitheltypen und ihre symmetrische Verteilung sprechen dafür, daß die Epitheljen an Ort und Stelle entstanden sind und keine weitere Ausdehnung erfahren haben. Demnach können die Dysplasien und die Carcinomata in situ, deren Lokalisation sich deutlich unterscheidet, auch keine aufeinanderfolgenden Stufen der gleichen Entwicklung darstellen. Die Lokalisation der verschieden differenzierten pathologischen Epithelformen hängt davon ab, auf welchem Boden es zur Entwicklung der Atypie gekommen ist: Je höher differenziert ein atypisches Plattenepithel ist, um so wahrscheinlicher ist es durch Umbau von Plattenepithel entstanden — entweder eines originären Plattenepithels oder eines Plattenepithels, das zunächst durch einfache Metaplasie im Drüsenbereich entstanden ist. Je geringer die Differenzierung eines atypischen Epithels ist, um so wahrscheinlicher ist hingegen seine Genese unmittelbar durch atypische Plattenepithelmetaplasie.
