Summary
The case report of a 28-year-old woman, para II, who came to us with erythroblastosis fetalis, severe pre-eclampsia and with bilateral, multiple lutein cysts is presented. This is only the tenth case observed. Hydrops fetalis et placentae was present, with a HCG-excretion level of 120,000 IU HCG/l, increasing to 320,000 IU, then sharply decreasing immediately after the expulsion of the hydropic fetus. Both ovaries reacted to this excessive stimulation by the formation of polycystic tumors, predominantly multiple lutein cysts. The spontaneous rupturing of these cysts and the resultant intra-abdominal hemorrhage led to peritoneal irritation and to the accumulation of more than 20 liters of ascites. Due to the extremely serious condition of the patient the ovaries had to be removed.
The discussion of the relationship between HCG and the growth of lutein cysts shows that high levels of HCG in hydrops fetalis et placentae occur only after breakdown of the feto-maternal metabolism. The development of lutein cysts then follows.
In contrast to the generally accepted opinion that the high levels of HCG are due to over-production by the trophoblast cells within the hydropic hyperplastic placenta, we discuss the theory that the cessation of feto-maternal metabolism is the significant causative factor. Our opinion is based on a comparison of placental structure in all those conditions in which HCG levels are high. The only common factor in all these cases was the immature, damaged or non-existent feto-maternal metabolism.
The relationship of HCG to toxemia of pregnancy and to lutein cysts as well as the therapy of this type of cystic tumor is discussed.

Es wird über eine 28jährige II.-Para berichtet, die in der 30. Schwangerschaftswoche wegen Erythroblastose, schwerer Präeklampsie und doppelseitigen Luteincysten zur Behandlung kam. Es ist dies der zehnte uns bekannte Fall. Es bestand das volle Bild des Hydrops fetus et placentae mit einer HCG-Ausscheidung von 120000 IE HCG/l, die schließlich bis auf 320000 IE anstieg, um erst nach Ausstoßung der hydropischen Frucht steil abzufallen. Beide Ovarien reagierten auf die exzessive Stimulierung mit Bildung polycystischer Tumoren unter Dominieren multipler Luteincysten. Spontane Rupturen der Cystenwände und Blutungen in die Bauchhöhle führten zur peritonealen Reizung mit Bildung von über 20 Litern Ascites, was wegen des lebensbedrohlichen Zustandes zur Exstirpation der Ovarien zwang.
Die Diskussion der Beziehungen HCG zu Luteincystenentstehung zeigte, daß die hohe HCG-Überschwemmung bei Hydrops foetus et placentae nach Zusammenbruch des fetomaternalen Stoffwechsels auftritt und die Ausbildung der Luteincysten nachfolgt.
Der vorherrschenden Ansicht, daß die hohe HCG-Ausscheidung in einer Mehrproduktion des Trophoblasten in der hydropisch-hyperplastischen Placenta ihre Ursache habe, wird die Hypothese des pathogenetisch entscheidenden Zusammenbruchs des fetomaternalen Metabolismus gegenübergestellt. Unsere Ansicht basiert auf der vergleichenden Betrachtung der Placentastrukturen bei Zuständen hoher HCG-Ausscheidung, denen gemeinsam nur der unreife oder gestörte oder fehlende fetomaternale Stoffaustausch ist.
Abschließend wird die Beziehung HCG-Spätgestose-Luteincysten und die Therapie derartiger Cystombildungen diskutiert.
