Is hysterectomy too radical a procedure for treatment of carcinoma in situ of the uterine cervix?

Summary

Several investigators have recommended adequate conization with subsequent regular follow-up as treatment of choice for carcinoma in situ of the uterine cervix. In this study we tried to get a follow-up as far as possible on 257 patients with squamous dysplasia or carcinoma in situ of the cervix. We found, that the proposed more conservative approach can be recommended in some carefully selected cases. However, we do not feel, that it should be employed generally as the treatment of choice since the absolutely necessary adequate long term follow-up on these patients is in many cases not possible.

Zusammenfassung

Zur Behandlung des Carcinoma in situ der Portio wurde von verschiedenen Untersuchern eine ausreichende Konisation mit anschließenden regelmäßigen Nachuntersuchungen als Methode der Wahl empfohlen. Wir versuchten das weitere Schicksal von 257 Patientinnen mit Dysplasie oder Carcinoma in situ der Portio so lange wie möglich zu verfolgen. Wir fanden, daß die vorgeschlagene konservative Behandlungsweise in sorgfältig ausgewählten Einzelfällen durchaus zu empfehlen ist. Sie als Methode der Wahl anzuwenden halten wir nicht für richtig, da die unbedingt notwendigen regelmäßigen Nachuntersuchungen in vielen Fällen nicht durchgeführt werden können.
