Summary
Following cesarean section a 23-year-old para IV developed a severe purulent necrotizing endomyometritis with perforation of the uterus, fibrinous purulent peritonitis and shock. The patient died in a circulatory collaps.
Chemical investigations of the protein component of the collagen of the puerperal uterus compared to normal uterine tissue of a nullipara revealed a marked denaturation of the collagen and an increased liberation of the soluble fraction of the collagen. This increased decomposition of collagen or the decreased stability of the collagen respectively, are assumed to be the cause of the spontaneous perforation of the uterus.
Zusammenfassung
Bei einer 23jährigen IV. para entwickelte sich nach Sectio caesarea eine schwere eitrige nekrotisierte Endomyometritis mit Uterusperforation, fibrinös eitriger Peritonitis und Schock. Die Patientin starb im toxischen Kreislaufversagen. Chemische Untersuchungen des Kollagenproteins des puerperalen Uterus im Vergleich mit normalem Uterusgewebe einer Nullipara ergaben eine starke Denaturierung des Kollagens sowie eine vermehrte Freisetzung des löslichen Kollagenanteils. Dieser erhöhte Kollagenabbau bzw. die verminderte Stabilität des Kollagens werden im vorliegenden Fall als Ursache für die spontane Uterusperforation angesehen.
