Summary
The pulmonary ventilation of 62 women (41 pregnant-, 10 post partum-, 11 control cases) was studied by means of bodyplethysmography. The gestation group was subdivided into group “A” 8–20 weeks of pregnancy, group “B” 20–28 weeks and group “C” 28–40 weeks of gestation. The study has the following result: During pregnancy occurs 1. a significant lowering of total lung capacity due to decrease of the exspiratory reserve volume and the functional residual capacity, 2. a decrease of maximal breathing capacity (maximal voluntary ventilation) whereas the forced exspiratory volume in percent of vital capacity volume (FEV1.0) remained unchanged and 3. an increase of the resistance (p=5%). The maximum of these alterations was found in the 26–28 week of pregnancy. The correlation between the decrease of functional residual capacity and increase of resistance is gestation-specific; the group of the control cases does not show such a relation. The above described changes of pulmonary ventilation during gestation do not lead to disturbances of arterialization of the arterial blood since the lung perfusion during pregnancy and the alveolar ventilation are increased whereby the lowering of the pulmonary ventilation is compensated.
Zusammenfassung
Bei 62 Frauen (41 Schwangere, 10 Wöchnerinnen, 11 Kontrollfälle) wurden mit Hilfe der Ganzkörperplethysmographie statische und dynamische Ventilationsgrößen untersucht. Die Schwangeren wurden in 3 Gruppen eingeteilt: Gruppe A 8.–20. Schwangerschaftswoche, Gruppe B 20.–28. Schwangerschaftswoche, Gruppe C 28.–40. Schwangerschaftswoche. Die Untersuchungen hatten folgende Ergebnisse:
Während der Schwangerschaft kommt es 1. zu einer signifikanten Erniedrigung der Totalkapazität. Sie ist bedingt durch die Abnahme des exspiratorischen Reservevolumens und der funktionellen Residualkapazität, 2. zu einer Abnahme des Atemgrenzwertes, wobei die relative Einsekundenkapazität unverändert bleibt und 3. zu einer statistisch auffälligen (p=5%) Erhöhung des Atemwegswiderstandes.
Die maximalen Veränderungen liegen in der 26.–28. Schwangerschaftswoche (zusammenfassende Darstellung Abb. 12).
Es besteht eine Korrelation zwischen der Abnahme der funktionellen Residualkapazität und der Zunahme des Atemwegswiderstandes; die Veränderungen sind als schwangerschaftsspezifisch anzusehen, da bei der Gruppe „Nicht-Schwangere“ keine Korrelation besteht. Da gleichzeitig während der Schwangerschaft die Lungendurchblutung und die alveoläre Ventilation (Belüftung) erhöht sind, werden Einschränkungen, die bei der Mehrzahl der statischen und dynamischen Ventilationsgrößen in der Schwangerschaft beobachtet werden z. T. kompensiert.
