Summary
The concentrations of D-(−)-3-hydroxybutyrate and acetoacetate are measured by enzymic method of Bergmeyer and Bernt in venous blood of pregnant women from 36. to 40. week and during labour as in umbilical vene at once after ligating the cord. For measuring the raised concentrations during labour the method had been so modified that 0.2–10.0 mg% substrate could be estimated routinely in 2 ml blood-volume.
The antiketogenic effect of xylitol has been tested by infusion of 500 ml xylitol 10% in half-isotonic electrolyte-solution.
Former results of health non-pregnant women had by individual constant keto-body level a difference in collected measurements of more than one tenth potency.
In late pregnancy our statements show an elevation of blood-level about 80% to 0.69 mg% BHB and 0.31 mg% AcAc, likewise of the quotient from normal 2.2 to 3.2.
During labour the results increase with likewise large individual differences in relation to the duration of birth during the first seven hours to results about 10 mg% BHB and 3 mg% AcAc, during that time after the results increase slowly only.
The quotient increases only a little over that in late pregnancy and decreases after delivery with sinking substrate concentrations for a short time under the normal level of non-pregnant women.
The results of umbilical vene-blood amount to 50% of maternal concentrations and are in no dependence of their highness. The fetal quotient lies constantly a little under the maternal one.
The high keto-body concentrations measured during tedious labour can drop by 500 ml xylitol 10% in half-isotonic electrolyte-solution to results like labours of normal duration.
Furthermore in maternal as fetal blood resulted a change of concentration in favour of the reduced partner.
Zusammenfassung
D-(−)-3-Hydroxybutyrat- und Acetoacetat-Konzentrationen im Venenblut Schwangerer in der 36.–40. Woche und unter der Geburt sowie in der Vena umbilicalis bei der Abnabelung wurden nach der enzymatischen Methode von Bergmeyer und Bernt bestimmt. Zur Messung der erhöhten Werte unter der Geburt wurde die Methode so modifiziert, daß Konzentrationen von 0,2–10,0 mg-% Substrat aus 2 ml Vollblut routinemäßig erfaßt wurden.
Die antiketogene Wirkung des Xylit wurde durch Infusion von 500 ml Xylit 10% in halbisotoner Elektrolyt-Lösung (Tutofusin® EX 10) geprüft.
Frühere Ergebnisse bei gesunden Nichtschwangeren hatten bei konstantem individuellen Ketonkörperspiegel in kollektiven Messungen Streuungen von mehr als einer 10er Potenz gezeigt. In der Spätschwangerschaft besteht nach unseren Befunden eine Erhöhung der Blutwerte um ca. 80% auf 0,69 mg-% BHB und 0,31 mg-% AcAc sowie ein Anstieg des Quotienten von normal 2,2 auf 3,2.
Unter der Geburt steigen die Werte mit ebenfalls großer individueller Streuung in Abhängigkeit von der Geburtsdauer während der ersten 7 Std auf Werte um 10 mg-% BHB und 3 mg-% AcAc, um danach nur noch langsam anzuwachsen. Der Quotient steigt geringfügig über den Wert der Spätgravidität und fällt nach der Entbindung mit sinkenden Substratkonzentrationen kurzfristig unter den Normalwert der Nichtschwangeren.
Die Blutwerte der Vena umbilicalis betragen etwa 50% der mütterlichen Konzentrationen, unabhängig von deren Höhe. Der fetale Quotient liegt stets geringfügig unter dem mütterlichen.
Die hohen Ketonkörperkonzentrationen, wie sie unter prolongierten Geburten gemessen wurden, ließen sich in den geprüften Fällen mit 500 ml Xylit 10% in halbisotoner Elektrolytlösung auf Werte senken, wie sie bei normal verlaufenden Geburten auftreten. Im mütterlichen wie fetalen Blut resultierte ferner eine Konzentrationsverschiebung zugunsten des reduzierten Partners.
