Studies on the solubility of skin collagen in scleroderma and the effect ofd-penicillamine

Summary

Biopsy specimens were taken from 18 patients with generalised and circumscribed scleroderma and from 8 control subjects. Solubility of collagen of affected and unaffected skin areas was studied. In comparison with other methods, the amount of dissolved material obtained by use of 8.0 M urea was ten times larger. The solubility of collagen is higher in scleroderma than in normal skin. Long-time treatment withd-Penicillamine normalized this collagen solubility pattern. It is suggested that in scleroderma a disturbance of collagen crosslinking is an important factor.

Zusammenfassung

An Biopsiematerial von 18 Patienten mit diffuser (progressiver) und circumscripter Sklerodermie und 8 Kontrollpersonen wurde das Löslichkeitsverhalten von Kollagen klinisch gesund erscheinender und erkrankter Hautareale untersucht. Durch die Anwendung von 8,0 M Harnstoff konnte die Ausbeute an gelöstem Material gegenüber anderen Verfahren etwa verzehnfacht werden. Die Löslichkeit nimmt in der Reihenfolge gesunde Haut < klinisch nicht befallene Haut < erkrankte Haut zu. Nach langdauernder Behandlung mitd-Penicillamin ändert sich das krankhafte Kollagenlöslichkeitsverhalten im Sinne einer Normalisierung. Die vorliegenden Befunde sprechen insgesamt dafür, daß bei Sklerodermie die Kollagenquervernetzung gestört ist.
