The fluorescent microscopical demonstration of pigment producing skin tumors after falck-hillarp compared to the normal light microscopical picture

Summary

By using the Falck-Hillarp fluorescence method, melanin producing cells can be shown in junction nevi, compound nevi, melanosis circumscripta praeblastomatosa Dubreuilh and in malignant melanoma. The same cells, having been stained with Hematoxylin-Eosin, are reidentified and compared with the fluorescent microscopical picture. Common structures, as well as characteristic differences between melanocytes and nevus cells are demonstrated. The typical cell formations in malignant melanoma appear clearly as nests or strands. Because of their high fluorophor content, they contrast sharply against the cellular infiltrate. It is just as easy to differentiate normal epidermal cells from melanoma cells, which have become separated and are then found as single cells between the keratinocytes. Melanosis circumscripta praeblastomatosa Dubreuilh differs by fluorescing cells, which seem to lie in no fixed order in the epidermis. The malignant cells which grow there are spindle-shaped. Astrocytic cells with long processes can also occur in small tumor cell nests.

Zusammenfassung

Melanin bildende Zellen werden mit der Fluorescenzmethode nach Falck-Hillarp in Junctions- bzw. Compoundnaevi, der Melanosis circumscripta praeblastomatosa Dubreuilh und in malignen Melanomen dargestellt. Die gleichen Zellen werden nach Anfärben mit Hämatoxylin-Eosin wieder identifiziert und mit dem fluorescenzmikroskopischen Bild verglichen. Gemeinsame Strukturmerkmale, sowie charakteristische Unterschiede zwischen Melanocyten und Naevuszellen werden aufgezeigt. In malignen Melanomen treten die typischen Zellformationen in Form von Nestern oder Zellsträngen klar hervor und heben sich durch ihren hohen Gehalt an Fluorophoren scharf von den Infiltratzellen ab. Ebenso leicht ist die Unterscheidung von normalen Epidermiszellen und nach außen abwandernden Melanomzellen, die dann als Einzelzellen zwischen den Keratinocyten zu finden sind. Die Melanosis circumscripta praeblastomatosa Dubreuilh zeichnet sich durch scheinbar ungeordnet in der Epidermis liegende fluorescierende Zellen aus. Die daraus entstehenden infiltrierend wachsenden Zellen sind spindelförmig. In kleinen Tumorzellnestern können hier auch astrocytäre Zellen mit langen Ausläufern vorkommen.
