Intracytoplasmic ultrastructures in peripheral blood cells in lupus erythematosus

Summary

Buffy coats of peripheral blood of different manifestations of lupus erythematosus were studied by electron microscopy for the existence of the so-called viruslike intracytoplasmic tubular structures. They could always be demonstrated in the blood leukocytes of systemic lupus erythematosus independent of duration, severity and prior medical treatment of the disease, but were completely absent in chronic discoid lupus erythematosus. So far these inclusions have been observed only in the active dermal lesions of the chronic lupus erythematosus whereas in various organs involved in systemic lupus erythematosus contained these structures. Their appearence in blood cells can therefore be interpretated as an early signal of visceral manifestation of the illness, which clinically is yet undetectable. Corresponding investigations in other systemic diseases as well as in healthy person were negativ.

Zusammenfassung

Von verschiedenen Erscheinungsformen des Erythematodes wurden Leukocytenkonzentrate des peripheren Blutes auf das Vorkommen sog. „virusähnlicher“ tubulärer intracytoplasmatischer Strukturen elektronenmikroskopisch untersucht. Während sie in Blutleukocyten bei visceralem Erythematodes, unabhängig von Dauer, Schwere und Vorbehandlung der Erkrankung, regelmäßig nachweisbar waren, konnten sie bei chronisch integumentalem Erythematodes nicht gefunden werden. Diese Zellstrukturen wurden beim rein integumentalen Erythematodes bisher nur in frischen Hautläsionen, beim visceralen Erythematodes dagegen in verschiedenen krankheitsbeteiligten Organen beobachtet. Ihr Auftreten in Zellen des peripheren Blutes kann somit als frühes Zeichen einer bereits eingetretenen, klinisch aber noch nicht faßbaren Generalisierung der Krankheit aufgefaßt werden. Entsprechende Untersuchungen bei anderen Systemerkrankungen sowie an einer gesunden Kontrollperson fielen negativ aus.
