Dependency, social isolation and mental health in a disabled population

Summary

The general hypothesis tested was that the degree of physical limitation, the dependency status resulting from that limitation and social isolation each have a negative effect upon the mental health of an impaired individual. The hypothesis was tested in a study population of former applicants for social security disability benefits. Mental health was measured through the use of an index composed of six psychiatric symptoms which correlated highly with clinically diagnosed mental illness in another context. Physical limitation was measured by the degree of physical mobility of study subjects. Dependency was measured by the need for the help of others required in personal care as well as in terms of economic independence. Isolation was measured with respect to the degree of social contact that study subjects had with others on a visiting basis. Physical limitation, dependency and social isolation were all found to be associated with poor mental health. Control tables suggested that each exerted an independent influence on the mental health of disability claimants included in this analysis.

Résumé

On a examiné l'hypothèse générale selon laquelle le degré d'atteinte physique, le status de dépendance résultant de cette atteinte et l'isolement social ont tous un effet négatif sur la santé mentale d'un individu affaibli. Cette hypothèse a été examinée dans une étude de la population composée par d'anciens candidats aux secours pour handicapés de la sécurité sociale. La santé mentale a été mesurée à l'aide d'un index composé de six symptômes psychiatriques qui étaient en haute corrélation avec les maladies mentales diagnostiquées cliniquement dans un autre contexte. L'atteinte physique a été mesurée selon le degré de mobilité physique des sujets de l'étude. La dépendance a été mesurée selon le besoin de l'aide d'autrui pour les soins personnels et en termes d'indépendance économique. L'isolement a été mesuré par rapport au degré du contact social que les sujets de l'étude entretenaient avec autrui, basé sur les visites. On a trouvé que l'atteinte physique, la dépendance et l'isolement social étaient chacun associés à une santé mentale déficiente. Des tableaux de contrôle ont permis de supposer que chacun exerce une influence indépendante sur la santé mentale des requérants à l'invalidité dont il est question dans cette analyse.

Zusammenfassung

Es wurde die allgemeine Hypothese untersucht, daß sowohl der Grad der physischen Behinderung wie der Abhängigkeitszustand, der aus jener Behinderung und sozialen Isolation herrührt, eine negative Wirkung auf die seelische Gesundheit eines behinderten Menschen haben. Die Hypothese wurde bei einer Untersuchungspopulation von früheren Bewerbern für soziale Wohlfahrtsleistungen getestet. Seelische Gesundheit wurde mittels eines Index gemessen, der aus sechs psychiatrischen Symptomen bestand, die hoch mit der in anderem Zusammenhang klinisch diagnostizierten seelischen Krankheit korrelierten. Die physische Behinderung wurde anhand des Grades der physischen Beweglichkeit der untersuchten Personen gemessen. Die Abhängigkeit wurde am Bedarf an persönlicher Fürsorge wie an der ökonomischen Unabhängigkeit gemessen. Isolation wurde in Hinsicht auf den Grad von sozialem Kontakt gemessen, den die Probanden mit anderen auf Besuchsbasis hatten. Physische Behinderung, Abhängigkeit und soziale Isolation zeigten sich alle in Verbindung mit schlechter seelischer Gesundheit. Kpntrolltabellen lassen vermuten, daß in dieser Untersuchung jede einen unabhängigen Einfluß auf die seelische Gesundheit von solchen ausübt, die Invaliditätsansprüche erheben.
