Summary
The coronary vasodilator drugs prenylamin and iproveratril were investigated with regard to their action on conductivity and on adrenergic β-receptors in the heart.
In isolated guinea-pig auricles, pronethalol and propranolol specifically inhibited the positive inotropic and chronotropic effects of adrenaline. Prenylamin, iproveratril and the antiarrhythmic drug ajmalin, on the other hand, primarily reduced strength and frequency of the spontaneous contractions; higher concentrations were needed to inhibit adrenaline.
The intravenous infusion of iproveratril in unanaesthetized rabbits prolonged the P–Q interval and lead to an atrioventricular block. The voltage of the ECG was reduced. Pronethalol, propranolol and prenylamin produced similar alterations in the ECG as ajmalin. In other experiments, the drugs mentioned were given in doses comparable with regard to the observed changes in the ECG. In these doses ajmalin, prenylamin and iproveratril did not reduce isoprenaline tachycardia, whereas pronethalol and propranolol inhibited it in much lower doses.
Propranolol completely prevented the depletion of cardiac glycogen in rats produced by isoprenaline. Ajmalin, prenylamin and iproveratril slightly increased glycogen content in comparison to the controls while pronethalol was ineffective.
The conclusion from these results was that prenylamin and iproveratril inhibit conduction in the heart by a quinidine-like action without producing an appreciable adrenergic β-blockade.
Zusammenfassung
Die Untersuchungen gingen von der Frage aus, ob die Coronardilatatoren Prenylamin und Iproveratril eine adrenerge β-Blockade am Herzen erzeugen.
Am isolierten Meerschweinchenvorhof hemmten Pronethalol und Propranolol die positiv inotrope und chronotrope Wirkung von Adrenalin in spezifischer Weise. Die Spontantätigkeit wurde erst durch wesentlich höhere Konzentrationen vermindert. Bei Gabe steigender Konzentrationen von Prenylamin, Iproveratril oder Ajmalin dagegen trat zunächst eine negativ inotrope und chronotrope Eigenwirkung auf; erst höhere Konzentrationen hemmten auch die Wirkung von Adrenalin.
Bei i.v. Infusion an wachen Kaninchen verlängerte Iproveratril die Überleitungszeit bis zum atrioventrikulären Block und verminderte die Spannung im EKG. Pronethalol, Propranolol und Prenylamin verursachten ähnliche EKG-Veränderungen wie Ajmalin. In Dosen, die bereits zu deutlichen EKG-Veränderungen führten, hemmten Ajmalin, Prenylamin und Iproveratril nicht die Tachykardie nach Injektion von Isoprenalin, während Pronethalol und Propranolol noch in wesentlich geringeren Dosen wirksam waren.
Propranolol verhinderte vollständig die Verarmung des Herzens von Ratten an Glykogen durch Isoprenalin. Ajmalin, Prenylamin und Iproveratril erhöhten den Glykogengehalt im Vergleich zu den Kontrollen nur geringfügig; Pronethalol war unwirksam.
Aus den Ergebnissen wurde geschlossen, daß Prenylamin und Iproveratril am Herzen die Erregungsausbreitung hemmen im Sinne einer chinidinähnlichen Wirkung, aber keine nennenswerte adrenerge β-Blockade erzeugen.
