Untersuchungen zur Ursache des negativen interstitiellen Gewebsdruckes (Guyton-Kapsel)

Investigations on the origin of the negative interstitial pressure (Gyton capsule)

Summary

In 30 rats 54 perforated capsules were implanted subcutaneously and the capsule fluid was analyzed 20–30 days postoperatively for Na+, K+, Cl and the protein content.

Compared with blood plasma the protein content was lower than plasma protein the average difference being 2.6 g/100 ml. Na+ and Cl concentrations were decreased by 4.5 mval/kg H2O and 5.0 mval/kg H2O, respectively.

Na+ and K+ values showed approximately a Gibbs-Donnan distribution.

From the distribution of the electrolytes a difference in osmotic pressure between plasma and capsule fluid space could be calculated ranging from 1–7 cm H2O (mean 2.63 cm H2O). In 15 capsules the negative pressures in the capsule fluid space was measured directly. They correlated statistically significant with the calculated differences in osmotic pressure.

It appears therefore that the pressure in the capsule fluid space mainly depends on the lowered protein concentration which in turn determines the distribution of the electrolytes.

Zusammenfassung

Bei 30 Ratten wurden insgesamt 54 Kapseln implantiert und der Kapselinhalt 20 und 30 Tage post op. auf die Elektrolyte Na+, K+ und Cl und den Eiweißgehalt hin untersucht.

Im Vergleich zum Plasma waren im Mittel die Konzentrationen des Na+ um 4,5 mval/kg H2O, die des Cl um 5,0 mval/kg H2O und der Eiweißgehalt um 2,6 g/100 ml erniedrigt.

Die Na+- und K+-Werte zeigten annähernd eine Gibbs-Donnan-Verteilung zwischen Blutplasma und Kapselflüssigkeit.

Aufgrund der Elektrolytverteilung konnten osmotische Druckdifferenzen zwischen 1–7 cm H2O (Mittel 2,63 cm H2O) errechnet werden, Kapseldruck negativ gegen Barometerdruck.

Direkt gemessene, negative Kapselinnendrucke korrelierten (r=0,515,p≤0,05) mit den errechneten osmotischen Druckdifferenzen.

Aus den Ergebnissen konnte geschlossen werden, daß es sich bei dem Kapselinhalt um eine relativ eiweißreiche Flüssigkeit handelte, wobei der Unterschied der Eiweißkonzentrationen seinerseits die Ionenverteilung zwischen Plasma und Kapselflüssigkeit bestimmte.
