Summary
Epidermal changes in human allergic and primary irritant contact dermatitis of 66 patch tests have been investigated electronmicroscopically in “longitudinal-sections”.

In the initial phase of the allergic contact-dermatitis an intercellular edema in the lower parts of the epidermis is formed, the number of desmosomes is reduced on account of a dissolution of the interdesmosomal areas which is called “microacantholysis”. The basement-membrane becomes dissolved where inflammatory cells are infiltrating the epidermis. A dissociation of unaltered keratinocytes is caused by the increased intercellular edema. Severe changes leading to cell damage have not been observed. Only few epidermal cells show periphereal intracytoplasmatic edema.—In the advanced stage of the allergic reaction not only a decrease of the intercellular edema but also increased proliferative activity as well as atypical keratinization of the epidermis take place.

After application of 50% croton oil the first noticeable changes consist in an intercellular edema of the lower parts of the epidermis and in cytolytic degenerative events. The microscopically observed “vésiculette primordiale” is caused by an intraepidermal vesiculation after rupture of involved keratinocytes. Vesiculation due to separation of desmosomes (microacantholysis) has not been observed. Remants of desmosomes in the enlarged intercellular space are results of fractured cytoplasmic processus. The dissolution of the epidermis in primary irritant contact dermatitis is caused by cytolysis.

In 66 Excisaten wurden die epidermalen Veränderungen allergischer und toxischer Epicutantestreaktionen vom Menschen elektronenmikroskopisch im „Längsschnittbild“ untersucht.

In der Initialphase der allergischen Kontaktreaktion bildet sich ein intercelluläres Ödem in den unteren und mittleren Epidermisschichten aus, die desmosomalen Kontaktpunkte vermindern sich infolge Auflösung der interdesmosomalen Haftstellen (Mikroacantholyse), die Basalmembran löst sich an Durchtrittsstellen einwandernder Infiltratzellen partiell auf. Durch Zunahme des intercellulären Ödems kommt es zur Dissoziation weitgehend intakter Keratinocyten. Schwere, zum Zelluntergang führende Veränderungen treten nicht auf. Die Ausbildung eines intracytoplasmatischen Ödems und degenerative Veränderungen bleiben auf einzelne Zellen beschränkt. In der abklingenden Reaktion setzt nach Rückbildung des intercellulären Ödems nicht nur eine Proliferation, sondern auch eine atypische Verhornung der Epidermis ein.

Nach lokaler Applikation von 50%igem Crotonöl bestehen die ersten sichtbaren Veränderungen neben einem intercellulären Ödem der unteren Epidermisschichten in cytolytisch-degenerativen Vorgängen, die schon frühzeitig zur Zerstörung einzelner Zellen führen. Die lichtmikroskopisch sichtbare „vésiculette primordiale“ entsteht durch intraepidermale Hohlraumbildung infolge Platzen ödematisierter Keratinocyten. Spongiotische Vesikelbildung infolge Mikroacantholyse wird nicht beobachtet. Die frei im erweiterten Intercellularraum liegenden Desmosomenreste werden durch ein mechanisch bedingtes Abreißen der Cytoplasmafortsätze verursacht. Bei der toxischen Reaktion beginnt die Hohlraum- und Spaltbildung primär intracellulär. Die epidermale Kohärenzstörung entsteht durch Cytolyse.
