Summary

The current state of mental health care in Brazil, and the problems of estimating needs in the population and of planning psychiatric services, are reviewed against the national historical, sociodemographic and medical background. The situation in Brazil is characterized by a shortage of psychiatric care agencies, as well as of the statistical and epidemiological data required for planning purposes. The following steps are proposed as a programme of action to bring about a rapid and relatively economic improvement in this situation: (1) establishment of psychiatric and child psychiatric departments in general hospitals; (2) establishment of out-patient, day-patient and rehabilitation facilities in the local communities; (3) legislation to abolish discriminatory practices against former psychiatric patients, and to extend health insurance cover to after-care and rehabilitation services; (4) setting-up of a national commission to enquire into the situation of mental health care, to formulate plans for a comprehensive, community-based mental health service and to promote research in this field.
