Der Einfluß der sozialen Schicht auf Aufenthaltsdauer und Entlassung

In an investigation based on the regional psychiatric hospital at Gabersee, south-east Bavaria, the duration of stay among patients resident on 1.1. 1971 was found to be inversely related to their social class. The lower the social class, the higher was the proportion of patients hospitalized for more than two years. The proportion of patients discharged within four years of admission was higher among those from the upper and middle classes than for those from the lower social classes. The social class gradient found in this enquiry was similar to that reported by Myers and Bean (1968) from New Haven, although average duration of stay was much lower, and average discharge rates much higher, in the present sample than in the American study. The influence of social class on duration of hospital stay and on discharge rate was of the same order as those of sex and marital status. In this respect, psychiatric diagnosis and mode of hospital admission were more important for prognosis. The influence of social class on prognosis for hospital discharge diminishes with increasing length of hospital stay.

Zusammenfassung

Am Beispiel des Bezirkskrankenhauses Gabersee konnte für die am 1.1.1971 hospitalisierten Patienten gezeigt werden, daß der Anteil der psychisch Kranken mit einer Aufenthaltsdauer von über zwei Jahren umso höher war, je niedriger die soziale Schicht. Die Rate der nach 4 Jahren Entlassenen war — ähnlich wie in einer Follow-up Studie in New Haven — für Angehörige der Ober- und Mittelschicht höher als für Unterschichtpatienten. Der Einfluß der sozialen Schicht auf Aufenthaltsdauer und Entlassung war in etwa so hoch wie der von Geschlecht und Familienstand; hingegen kommt der Art der Diagnose und dem Einweisungsmodus eine sehr viel größere Bedeutung zu. Mit zunehmender Dauer der Hospitalisierung nimmt der Einfluß der sozialen Schichtzugehörigkeit auf die Prognose bezüglich der Entlassung ab.
