The community mental health centers program in the U.S.: A new system of mental health care

Summary

A new system for delivering mental health services in the United States is now being developed. The system depends on the establishment of a new organizational form, the comprehensive community mental health center. The program of such a center offers a comprehensive range of available and accessible services including as a minimum inpatient, outpatient, partial hospitalization and emergency services as well as consultation and education to community organizations and professional workers. Continuity of patient care is maintained for as long as required by the patient. Federal funds in addition to State and local monies support the construction of new facilities and certain operating costs. The center program is tailored to the needs of each community and catchment area and coordinates its activities with other public and private agencies and practitioners. The program is still too new to evaluate but experience thus far indicates ready public and professional acceptance and improved patient services.

Un nouveau système de répartition des services de santé mentale est développé maintenant aux Etats-Unis. Ce système repose sur l'établissement d'une nouvelle forme d'organisation: le centre de santé mentale communautaire étendu. Le programme d'un tel centre offre une série très complète de services valables et accessibles comprenant au minimum des services clinique, ambulatoire, d'hospitalisation partielle et d'urgence, ainsi qu'un centre de consultation et d'éducation destiné aux organisations de la communauté et aux travailleurs professionnels. La continuité des soins au patient est maintenue aussi longtemps que celui-ci le demande. Des crédits fédéraux, en plus des fonds locaux et de l'Etat, supportent les nouvelles constructions et certains frais d'opération. Le programme du centre est adapté aux besoins de chaque communauté et coordonne ses activités avec celles d'autres institutions publiques et privées et celles des praticiens. Le programme est encore trop nouveau pour qu'il soit possible d'en évaluer la valeur, mais l'expérience montre jusqu'à présent qu'il est bien accueilli dans le public et dans les milieux professionnels et que les services de malades se sont améliorés.

In den USA wird gegenwärtig ein neues System für die Verbesserung der Dienste für die seelische Gesundheit entwickelt und angewandt. Das System hängt von der Errichtung einer neuen Organisationsform, dem „psychiatrischen Gemeindezentrum“ (community mental health center), ab. Das Programm eines solchen Zentrums bietet eine umfangreiche Skala von verfügbaren und leicht zugänglichen Diensten an, die sowohl als Minimum Vollhospitalisierung, ambulante Behandlung, Teilhospitalisierung und Notfalldienste wie auch Beratung und Ausbildung für Gemeindeorganisationen und für professionelle Mitarbeiter einschließen. Kontinuität bei der Patientenversorgung wird solange aufrechterhalten, wie es vom Patienten gefordert wird. Bundesstaatliche Fonds unterstützen, zusätzlich zu Staats- und Gemeindegeldern, den Ausbau neuer Anlagen und die Betriebskosten. Das Programm des Zentrums ist auf die Bedürfnisse jeder Gemeinde und jedes Einzugsgebietes zugeschnitten und koordiniert seine Aktivitäten mit anderen öffentlichen und privaten Einrichtungen und mit den praktizierenden Ärzten. Das Programm ist noch zu neu, um es in seinem Wert abzuschätzen, aber die Erfahrung zeigt bisher eine günstige Aufnahme bei der Öffentlichkeit und den Fachleuten sowie verbesserte Patientendienste.
