Summary
This study is based on a population of 100 patients presenting for the first time to an extramural psychiatric consultation service. The majority of patients were found to be neurotic, female, of middle social class, and were requesting psychotherapy to help resolve interpersonal difficulties. The response made by the service appeared to be appropriate to both the potentialities and needs of the patients. Nevertheless, as in the USA, an appreciable number of patients dropped out of treatment and it was not possible as in Lazare's study, to ascribe these to the inadquacies of the organisation's response to the patients demands. Every request for psychotherapeutic help engenders unconscious resistance by the patient, and it does not seem possible or desirable to offer an “answer for everything”. On the contrary the readiness with which care is given by an institution enhances the ambiguity and immaturity of the request for help, and may contribute substantially to non-completion of treatment.
Resumé
L'étude porte sur une population de 100 patients se présentant pour la première fois dans un centre de consultation de psychiatrie externe (l'Elan). Le profil de la majorité de ces patients se définit comme suit: public féminin névrosé, de haut niveau socio-culturel, faisant une demande de psychothérapie pour résoudre des difficultés qui seraient surtout d'ordre relationnel. Les réponses proposées: psychothérapies en général, semblent adaptées aux demandes des patients et à leurs possibilités. Néanmoins, tout comme aux USA, on constate un nombre important d'abandons de traitement pour lesquels on ne peut ici invoquer, comme le fait Lazare, une non adéquation de la réponse à la demande. Toute demande d'aide psychothérapique entraînant des résistances inconscientes chez le patient, il ne semble ni possible ni souhaitable d'offrir une “réponse à tout”. Au contraire, la facilité avec laquelle des soins en institution sont prodigués favoriserait l'ambiguité et la non-maturation de la demande, à l'origine du taux important d'abandons de traitement.
