Extensionsfraktur der Schädelbasis bei sturzhelmgeschütztem Kopf

Zusammenfassung

Es handelt sich um eine typische ringförmige Extensionsfraktur der Schädelbasis, entstanden bei extremer Retroflexion des Kopfes bei Zusammenstoß eines mit Sturzhelm versehenen Motorradfahrers mit einem Lastwagen. Diese Retroflexion ergab sich nicht durch Hebelwirkung der Wirbelsäule, wie in dem vonPatscheider beschriebenen Fall, sondern durch Anprall des sturzhelmgeschützten Kopfes selbst. Ohne die sturzhelmbedingte Übertragung des Stoßimpulses auf den gesamten Kopf wäre es möglicherweise nur zu einem Biegungsbruch des Schädeldaches an der Anprallstelle gekommen. In der einschlägigen Literatur ist unseres Wissens keine Mitteilung über einen derartigen Entstehungsmechanismus enthalten.

Summary

A typical ring-like extension fracture of the base of the skull due to extreme retroflexion of the head while using a crash helmet in a motorcyclist colliding with a truck is reported. This retroflexion was not due to a lever-like action of the vertebral column as described byPatscheider but resulted directly from impact of the head protected by the crash helmet. Without the use of the helmet the impact would have been transmitted over the entire head, possibly causing a fracture limited to the point of impact.

No other report relating a similar mechanism of injury was found in the pertinent literature at our disposal.
