Summary
This study concerns male patients first admitted in 1964 to the Rheinisches Landeskrankenhaus at Düsseldorf, the Psychiatrische Universitätsklinik at Düsseldorf, and the Alexianer Hospital at Neuß (where some patients from Düsseldorf are also sent). The male population of Düsseldorf was divided into two social classes — an upper and a lower — as a basis for comparing social stratification within specific illness groups. The difference between the proportions in these two classes provides clues about the relationships between hospitalisation for certain illnesses and social background. Five diseases appeared often enough to allow a comparison to be made. The general population was divided into 50.8% upper class and 49.2% lower class. A similar class division is found among older men suffering from mental disturbances in the Düsseldorf area and admitted to the two above mentioned hospitals in 1964 for the first time. Among alcoholics, and to some extent among schizophrenics and patients with psychoneurotic disorders there is a preponderance of lower class males, ranging from 63% to 67%. Among manic-depressive patients, however, upper class males account for 70% of first admissions. The correlation between the first admission rates from the 40 Düsseldorf districts in 1964, and the proportion of upper class males in those districts, is −0.12, which is not statistically significant. Presumably the city districts are socially not differentiated enough to provide a reliable comparison base.
Résumé
L'effectif des malades hommes reçus en première admission au Rheinisches Krankenhaus, Clinique psychiatrique universitaire de Düsseldorf et au Alexianer-Krankenhaus Neuß a été étudié au point de vue de la répartition des diagnostics selon les couches sociales; ce type de répartition a été ensuite comparé avec les résultats de l'ensemble de la population masculine de Düsseldorf. Dans l'ensemble, il n'y avait pas de différence en ce qui concerne les troubles psychiques de la sénescence. Les alcooliques en première admission venaient surtout des couches inférieures; pour les maniacodépressifs c'était le contraire. Les couches inférieures n'étaient que très légèrement plus atteintes de „réactions anormales“ ou de „schizophrénie“. - Il n'a pas été possible d'établir une corrélation entre le status social des différents quartiers de la ville de Düsseldorf et les hospitalisations y relatives. Peutêtre les différents quartiers ne sont-ils plus suffisamment distincts. - Pour commencer, l'étude parle de la littérature la plus récente à ce sujet. Dans la discussion on observe que l'étude ne permet pas de conclusions épidémiologiques générales. Il est toutefois possible de conclure, avec quelque restriction, à la relation qui existe entre les différents groupes de maladie et les couches sociales des hommes de Düsseldorf reçus en première admission la difficulté des enquêtes épidémiologiques a été rappelée.
Zusammenfassung
Das Krankengut der 1964 erstaufgenommenen Düsseldorfer Männer des Rheinischen Landeskrankenhauses Düsseldorf, Psychiatrische Universitätsklinik Düsseldorf, und des Alexianer-Krankenhauses Neuß wurde darauf durchgesehen, wie sich die sozialen Schichten einerseits auf die Kranken einer bestimmten Diagnose verteilen; dieser Verteilungstyp wurde andererseits mit dem in der Düsseldorfer Männer-Gesamtpopulation verglichen. Die psychischen Störungen des höheren Lebensalters verhielten sich im Vergleich indifferent. Die neuaufgenommenen Alkoholiker hatten einen Verteilungstypus mit Bevorzugung der unteren Schichten, die Manisch-Depressiven umgekehrt. Weniger hervorgehoben war die Bevorzugung der unteren Schichten durch die Gruppen „Abnorme Reaktion“ und „Schizophrenie“. - Eine verwertbare Korrelation zwischen Sozialstatus der einzelnen Düsseldorfer Stadtteile mit den zugehörigen Hospitalisationsraten ließ sich kaum ermitteln. Vielleicht sind die einzelnen Stadtteile dafür nicht mehr profiliert genug. - Eingangs wurde das einschlägige neueste Schrifttum referiert. In der Diskussion wurde darauf hingewiesen, daß die Studie keine allgemeinen epidemiologischen Rückschlüsse erlaubt. Mit Vorbehalt kann man jedoch von der sozialen Schichtenverteilung in den verschiedenen Krankheitsgruppen (der 1964 erstaufgenommenen Düsseldorfer Männer) auf die Beziehungen schließen, die zwischen den sozialen Niveaus und bestimmten psychiatrischen Krankheiten walten. Die Schwierigkeiten epidemiologischer Untersuchungen wurden erörtert.
