Summary
A controlled evaluation of a transitional social and vocational hospital-community rehabilitation program was carried out over a period of twenty months. Following initial comparison of Rehabilitation and traditional Continued Treatment, the Rehabilitation Program was expanded to all the chronic psychiatric wards. - Conclusions were that the present multiple ward hospital rehabilitation program is more effective than the former traditional services but not as effective as the smaller, more intense, initial experimental program. A most important aspect of this service is organized interagency cooperative management of patients in community sheltered employment. A deficiency in the present services is the lack of comparable community sheltered living facilities. As a result of this deficiency, it seems probable the period of hospitalization of many clients is being unnecessarily prolonged.
Résumé
Une évaluation contrôlée d'un programme de réadaptation transitoire social et professionnel a été effectuée pendant 20 mois dans une communauté hospitalière. Après une comparaison initiale de la réadaptation et du traitement traditionnel continué, le programme de réadaptation fut étendu à toutes les divisions de malades mentaux chroniques. - La conclusion fut que l'actuel programme de réadaptation en divisions hospitalières multiples est plus efficace que les anciens services traditionnels, mais moins efficace que le programme initial, plus bref, plus intense. Un des aspects très importants de ce service est la direction coopérative interagences des patients travaillant dans une communauté protégée. L'absence d'une communauté protégée aux conditions de vie comparables est une lacune. Le résultat de cette lacune est que probablement la durée de l'hospitalisation de beaucoup de patients est inutilement prolongée.
Zusammenfassung
Ein zeitlich auf 20 Monate begrenztes, soziales und berufliches Rehabilitations-Programm wurde mit den Erfolgen der traditionellen Behandlung bei stationären Patienten eines psychiatrischen Krankenhauses verglichen. Nach einem ersten Vergleich zwischen der rehabilitativ orientierten und der traditionellen Dauerbehandlung wurde das Rehabilitations-Programm auf alle Abteilungen mit chronisch psychiatrischen Patienten übertragen. - Es ergab sich, daß das auf alle Abteilungen ausgedehnte Rehabilitations-Programm wirksamer war als die früheren traditionellen Behandlungsweisen; es war jedoch weniger wirksam als das anfänglich in kleinerem Umfang und intensiver durchgeführte experimentelle Programm. Entscheidend für den Erfolg ist die Zusammenarbeit aller befaßten Stellen mit jenen Institutionen, welche die geschützte Betätigung der Patienten in der Gemeinschaft tragen. Ein Mangel der beschriebenen Einrichtungen wird im Fehlen geschützter Wohnmöglichkeiten in der Gemeinschaft gesehen. Dies führt dazu, daß die Krankenhausverweildauer vieler Patienten unnötig verlängert wird.
