Zusammenfassung
Histologisch finden sich beim Keratoconus im zentralen Hornhautbereich die unterschiedlichsten Veränderungen: Das Epithel kann verdünnt, aber auch stark verdickt sein. Die Bowmansche Membran fehlt, ist rupturiert, verlagert, verdickt oder sogar verdoppelt. Das verdünnte Hornhautstroma ist regelrecht oder auch von zahlreichen Zellen durchsetzt. Die Descemet erscheint verdünnt oder auch maximal verdickt, eventuell durch straffes Bindegewebe ersetzt. Das Endothel ist regelrecht oder auch rarefiziert mit Kernresten oder fehlt auch ganz. Beim sog. „akuten“ Keratoconus kann in seltenen Fällen die (verdickte) Descemet intakt und nur das Hornhautendothel schwer geschädigt sein.
Summary
On histological examination, very different changes can be found in the central corneal area in keratoconus: There can be a thinned, but also a considerably thickened epithelium. Bowman's membrane can be missing, ruptured, displaced, thickened or even doubled. The thinned corneal stroma is either of regular appearance or also interspersed with numerous cells. Descemet's membrane may appear thinned, but also be of maximal thickness, perhaps even replaced by tight connective tissue. The endothelium is either regular or also rarefied with remnants of cellular nuclei or may even be absent at all. In the so-called “acute” keratoconus, Descemet's membrane may be intact in rare cases and only the corneal endothelium may show severe lesions.
