Summary
Correlation has previously been observed between the incidence of metal hypersensitivity (Ni, Cr, Co) and the incidence of infection after internal fixation of bone fractures. As there is no doubt that exposure to high concentrations of metal can cause allergy, metal levels (Cr, Ni, Mo, Co) were analyzed in implant contact tissues by ASS. Both the absolute concentration of the ions and the Ni: Cr ratio in the tissues adjacent to metal implants is greater in infected cases than in non-infected. Thus infection as a cause of allergy has to be considered. In view of the resulting problems perhaps consideration could be given to the use of a different alloy for metal implants.
Zusammenfassung
In vorangegangenen Untersuchungen konnte ein Zusammenhang zwischen Allergie gegen implantatspezifische Metalle (Ni, Cr, Co) und Infektmorbidität bei Patienten mit Osteosynthesen nach Frakturen festgestellt werden. Da kein Zweifel daran besteht, daß eine Exposition gegen hohe Metallkonzentrationen eine Allergie verursachen kann, wurden im Implantatkontaktgewebe Metallkonzentrationen (Cr, Ni, Mo, Co) mit der Atomabsorptionsspektroskopie bestimmt. Dabei ergab sich, daß sowohl die Nickel- und Chromkonzentrationen wie auch die Ni: Cr-Relation im Implantatkontaktgewebe infizierter Osteosynthesen größer ist als bei nicht infizierten. Es kann daher angenommen werden, daß die Infektion über die Erhöhung der Metallgewebekonzentrationen eine erhöhte Allergierate hervorruft. Angesichts der daraus folgenden Probleme stellt sich daher die Frage nach der Verwendung von Stählen mit verbesserter Korrosionsbeständigkeit auch in der Chirurgie.
