Dermatoses with Transepithelial Perforation

Summary

The histologic concept of epidermal elimination of particulate matter was formulated by Pinkus in 1954. Mehregan enlarged on this by recognizing its fundamental role in a variety of skin diseases. The importance of this epidermal reaction pattern in five “perforating” dermatoses is stressed. Also, it is shown that active participation of the corium and epithelio-mesenchymal interaction are required for the initiation of this mechanism. The histological differentiation and histogenesis of the presented dermatoses are discussed.

Zusammenfassung

Das von H. Pinkus 1954 formulierte biologische Konzept der epidermalen Elimination wurde in der Folge von Mehregan als grundlegendes Prinzip bei einer Reihe von heterogenen Hautkrankheiten erkannt. Die Bedutung dieses epidermalen Reaktionsmusters wird an Hand von fünf “perforierenden” Dermatosen demonstriert und sowohl die aktive Beteiligung des Koriums an diesem Mechanismus, sowie die Rolle epitheliomesenchymaler Wechselbeziehungen hervorgehoben. Es wird die histologische Differentialdiagnose und unterschiedliche Histogenese dieser Krankheitsgruppe diskutiert.
