Summary
Light and electronmicroscopic studies as well as a statistical analysis of the lympho-epithelial relationship in the gut are published. The following diseases have been investigated: nonspecific enteritis, Whipple's disease, idiopathic steatorrhea, ulcerative proctocolitis, Crohn's disease and carcinoma of the large intestine.
In the mucous membranes of the normal small intestine and nonspecific enteritis the number of interepithelial lymphocytes is 61.234±11.052 and 73.125±13.320, whereas in idiopathic steatorrhea it is 157.278±27.031. For both values the t-distribution by Student clearly shows significant differences.
In normal and inflamed mucous membranes of the rectum and in cancer of the large intestine the number of lymphocytes is found between 38.625±3.852 and 45.154±5.90, in mucous membranes of ulcerative proctocolitis the number is 106.609±16.309 and 113.315±28.026 and in Crohn's disease 180.909±35.863. The t-distribution again shows significant differences.
The lymphocytes are located intercellularly, predominantly in the infranuclear position. In normal as well as inflamed mucous membranes small lymphocytes are to be seen. A considerable number of these lymphocytes become activated in idiopathic steatorrhea, ulcerative proctocolitis and Crohn's disease. The cells increase in size and have more cytoplasmic organelles. Osmiophilic inclusion-bodies develop. The nucleus enlarges, often showing invagination of the nuclear membrane. The nuclear chromatin is finely distributed. A short local application of corticosteroids has no influence on the activated lymphocytes of the rectal mucosa. Electron as well as statistical results point to the possible immunopathological mechanisms in the above mentioned diseases. Applying the definition of Medawar's, the interepithelial lymphocytes are understood as immunocompetent cells.
Zusammenfassung
Lympho-epitheliale Beziehungen des Darmes werden an Hand von Biopsie- und Operationspräparaten licht- und elektronenmikroskopisch untersucht und statistisch ausgewertet. Im einzelnen handelt es sich um folgende Krankheitsbilder: unspezifische Enteritis, Morbus Whipple, idiopathische Steatorrhoe, Proctocolitis ulcerosa, Morbus Crohn und Colon-Carcinome.
Bei normaler Dünndarmschleimhaut, unspezifischer Enteritis und Morbus Whipple liegt die Zahl der interepithelialen Lymphocyten im Mittel zwischen 61,234±11,052 und 73,125±13,320, bei idiopathischer Steatorrhoe bei 157,278±27,031. Für beide Werte ergibt die t-Verteilung nach Student eine eindeutige Signifikanz.
Bei normaler und entzündlich gereizter Rectumschleimhaut und bei Colon-Carcinomen liegt die Lymphocytenzahl zwischen 38,625±3,852 und 45,154±5,90, bei Proctocolitis ulcerosa und Morbus Crohn bei 106,609±16,309 bzw. 113,315±28,062 und 180,909±35,869. Auch hier ergibt die t-Verteilung eindeutig signifikante Unterschiede.
Die Lymphocyten liegen im Epithelverband intercellulär und zumeist infranukleär. Es handelt sich bei normaler und entzündlich irritierter Mucosa um sog. kleine Lymphocyten. Ein erheblicher Teil erfährt bei idiopathischer Steatorrhoe, Proctocolitis ulcerosa und Morbus Crohn eine „Aktivierung“. Die Zellen werden größer. Die Organellenausstattung ist reichhaltiger. Es kommt zur Entwicklung osmiophiler Einschlußkörper. Die Zellkerne sind vergrößert, vielfach invaginiert und leptochromatisch. Eine kurzfristige, lokal-rectale Corticoidmedikation hat keinen Einfluß auf die „aktivierten“ interepithelialen Lymphocyten.
Ultrastrukturelle und statistische Befunde deuten auf pathogene Immunphänomene und-mechanismen im Ablauf verschiedener Enteropathien. Die interepithelialen Lymphocyten sind in diesem Zusammenhang als „immunkompetente“ Zellen im Sinne der umfassenden Definition Medawar's aufzufassen.
