Summary
The electron microscopic examination of 116 human renal biopsies from patients with proliferative glomerulonephritis has shown the following results: The electron dense deposits in the mesangial matrix or in the capillary basement membrane were considered as an evidence of a relatively fresh inflammatory process against the glomerular capillary loops. In persistent glomerulonephritis (postacute by Bohle, 1964) the deposited material was less electron dense. In cases of asymptomatic chronic glomerulonephritis no difference between the density of the deposits and that of the basement membrane could be observed.
In the nephrotic syndrome the electron dense deposits were more frequently observed than in the non-nephrotic form. The continous albuminuria induced the diffuse thickening of the capillary basement membrane, whereas the long-term hypertension caused the nodular thickening of the mesangial matrix.
The so-called “mesangial cell proliferation” in the persistent and chronic glomerulonephritis was usually accompanied by infiltration of lymphocytes and monocytes.
Zusammenfassung
An 116 menschlichen Nierenpunktaten wurden die feinstrukturellen Veränderungen bei proliferativer Glomerulonephritis untersucht. In der Mesangiummatrix und der Capillarbasalmembran wurden elektronendichte Ablagerungen gefunden, die als pathognomonisch für einen frischen entzündlichen Prozeß an den Glomerulumcapillaren gedeutet werden.
Bei persistierender Glomerulonephritis (postakute Form nach Bohle, 1964) war die Dichte des abgelagerten Materials im allgemeinen geringer. In Fällen von asymptomatischer chronischer Glomerulonephritis konnte überhaupt kein Dichteunterschied zwischen den Ablagerungen und der Basalmembran mehr festgestellt werden.
Beim nephrotischen Syndrom war die Menge der Ablagerungen erheblich größer als bei der nichtnephrotischen Form.
Das nephrotische Syndrom wird als Ausdruck eines entzündlichen Prozesses angesehen, der an den Glomerulumcapillaren abläuft.
Die anhaltende Albuminurie wird als Ursache der diffusen Verdickung der Basalmembranen gedeutet. Ein lange bestehender Hochdruck wird für die knötchenförmige Verdickung der Mesangiummatrix verantwortlich gemacht.
Die celluläre Proliferation bei der persistierenden und chronischen Glomerulonephritis ist häufig begleitet von Monocyten- und Lymphocyteninfiltraten.
