Summary
In normal human muscles some fibers show areas of local degeneration and regeneration. Initially these areas are limited to myofibrils. After lateral dissociation of filaments the affected sarcomers are stretched, the Z-bands become wider and desintegrate. The Z-material condenses into Z-rods, the filaments are dissolved without participation of lysosomes. Free ribosomes may appear to form new filaments. These filaments constitute atypical short fibrils without any cross striation. The formation of regular fibrils could not be identified. In extended regions of degeneration the fiber produces a sarcoplasmic cavity containing pycnotic nuclei, bundles of filaments, lysosomes, lipofuscin granules, swollen mitochondria and whorls of concentric membranes. In many myopathies rods and areas without myofibrils have been described. The reason for local fibrillar degeneration in normal muscle fibers is unknown.
The observed degenerations in myofibrils are considered as increasing dedifferentiations of parts of the muscle cell. Interruption of bridges between myosin and actin filaments, i.e. reduction of ATP-ase activity of myosin, is discussed to be the molecular basis of the morphological alterations in the myofibrils.
Zusammenfassung
In normaler menschlicher Muskulatur zeigen einzelne Fasern lokal begrenzte De- und Regenerationsherde. Diese beschränken sich zunächst auf die Fibrillen. Nach lateraler Dissoziation der Myofilamente werden die befallenen Sarkomere gedehnt, die Z-Streifen werden breiter und zerfallen. Das Z-Material kondensiert zu Z-Stäben, die Filamente lösen sich ohne Beteiligung von Lysosomen auf. Es können freie Ribosomen auftreten, die neue Filamente synthetisieren. Diese bilden kurze Fibrillen ohne Sarkomereinteilung. Eine eventuelle Neubildung regelrechter Sarkomere konnte nicht identifiziert werden. Ist der Degenerationsherd groß, entstehen myoplasma-erfüllte Höhlen innerhalb der Faser, die pyknotische Kerne, Filamentbüschel, Lysosomen, Lipofuscin, geschwollene Mitochondrien und Myelinfiguren enthalten. Z-Stäbe (rods) und fibrillenfreie Areale sind bei vielen Myopathien beschrieben worden. Der Grund ihres Auftretens in normaler Muskulatur ist unbekannt.
Die beobachtete Form einer einfachen Degeneration wird mit einer lokalen zunehmenden Entdifferenzierung der Muskelzelle gleichgesetzt. Als molekulare Ursache der morphologischen Veränderungen der Fibrillen wird die Zerstörung der interfilamentären Brücken, d.h. der ATPase-Aktivität des Myosins, diskutiert.
