A report is presented on the clinical picture and necropsy findings of a 30 year old women who showed severe destructive inflammation of the joints and juxta-articular, calcareous nodules and lipoids in primary chronic polyarthritis. Instead of the typical morphological changes there were foam cells in the synovia and as a manifestation of the generalized systemic involvement.

Neutral and acid mucoid substances were demonstrated in the foam cells and lipoids with increased free cholesterine as well as hydroxyapatite.

It is assumed the lesions described represent a peculiar morphological pattern of primary chronic polyarthritis. Very similiar lesions are seen in multicentric reticulohistiocytosis, however, they differ clearly in the clinical course, the histochemical findings lack of acid mucoid substances and the frequent giant cells.

Bericht über Klinik und Sektionsbefund einer 30jährigen Patientin mit schweren Gelenkdestruktionen und juxtaartikulären Lipoid-Kalkknoten bei primär chronischer Polyarthritis. An Stelle der typischen morphologischen Veränderungen war es im Synovialgewebe und bei der Begleitreaktion des Gesamtorganimus zur Ausbildung von Schaumzellen gekommen.

In den Schaumzellen ließen sich neutrale und saure Mucosubstanzen und in den gelenknahen Knoten Lipoide mit erhöhtem freien Cholesterin sowie Hydroxylapatit nachweisen.

Es wird angenommen, daß es sich bei der Erkrankung um eine besondere morphologische Verlaufsform der primär chronischen Polyarthritis handelt. Die differentialdiagnostische Abgrenzung gegenüber den sehr ähnlichen Veränderungen bei der multizentrischen Reticulohistiocytose ist gegeben durch die klinischen Befunde, die histochemischen Befunde (Fehlen der sauren Mucosubstanzen) und das Überwiegen von Riesenzellen bei der multizentrischen Reticulohisticytose.
