Summary
The analysis of the endocrinologically active tumours that produce amyloid discloses that amyloid is deposited in two forms:

1.
Deposition of amyloid between the tumour cells. The production of amyloid possibly occurs inside the epithelial cell.

2.
A strictly subendothelial deposition of amyloid.
The second form was seen in one of the insulinomas and is considered to be typical for this tumor, whereas the direct, nonsubendothelial form of deposition in thyroid cancer with the production of calcitonin is common for this tumor. In one of our insulinomas we observed both forms of deposition.
Amyloid seems to be produced by many tumours derived from cells of the “Helle Zellen” system. However, considering our results, we feel that various pathogenetic mechanisms might exist to induce an amyloid production. We think therefore that its formation is not dependent on a particular cell type but represents rather a reaction to an influence on the tumour cell from the outside.
We discuss other observations: 1. almost complete absence of A2-cells in the islets of Langerhans in one insulinoma case and 2. frequency of mast-cells in insulinomas.
Zusammenfassung
Die Analyse endokrin aktiver Tumoren mit Amyloidstroma hat ergeben, daß zwei Arten von Amyloidablagerung vorkommen:

Direkte Bildung oder Induktion der Produktion durch die Tumorzellen und Ablagerung des Amyloids zwischen den Zellen und außerhalb der Gefäße.

2.
Ablagerung im Bereich der subendothelialen Gefäßmembran.
Diese Form kommt bei einem unserer Insulom-Fälle allein vor und wird auch in der Literatur als typische und alleinige Form der Amyloidablagerung beschrieben. Hingegen ist ausschlie\lich die Variante 1 zu finden im Fall des C-Zell-Tumors der Schilddrüse. Im zweiten Insulom können subendotheliale Ablagerungen und Schollen innerhalb von Tumorzellgruppen wie beim medullären Schilddrüsencarcinom gesehen werden.
Amyloidproduktion mag zwar bei einer besonderen Gruppe endokrin aktiver Tumoren, besonders auch bei den Tumoren des Helle-Zellen-Systems typisch sein. Der Umstand, daß wir ganz verschiedene Bilder der Amyloidablagerung haben — eine subendotheliale und eine direkt mit den Tumorzellen zusammenhängende —, scheint uns ein wichtiges Indiz dafür, daß die pathogenetischen Mechanismen nicht einheitlich sind. Wir nehmen deshalb auch an, daß die Amyloidbildung von außen induziert wird und nicht an ein bestimmtes Zellsystem gebunden ist. Weiter können wir bestätigen, daß die endokrine Aktivität auch bei einem klinisch stummen Tumor histologisch nachweisbar sein kann, wenn Gegenspielerdrüsen zum Ausgleich der Hormonbalance eingesetzt werden.
