Summary
In histological studies of pathological changes of the human breast calcification was found in the retained secretion, in the epithelium, and in the stroma of the gland. Since the pathomorphogenesis of these calcifications is unknown, pathogenetic factors contributing to the calcification of the mammary gland were studied in experiments. The model of calciphylaxis (Selye) was used for the study. Tests were carried out in 70 Sprague-Dawley-rats. The calcium metabolism was increased by oral application of 1 mg dihydrotachysterol (sensitizer) on the first day of the test. As a second factor (challenger) experimental retention of milk was produced by ligature of some mammillae. In another experimental group, after application of dihydrotachysterol, a metabolic damage of cells was effected by intraperitoneal injection of ethionine (2% watery solution). Complete dosage of ethionine: 400–800 mg per kg animal. Histochemical proof of calcium deposits were obtained with the methods of Voigt and V. Kossa.
Results. 1. Dihydrotachysterol increases the secretory activity in the mammary gland of lactating rats and augments the calcium content of the secretion. Application of ethionine alone causes a greater precipitation of calcium in the secretion of the gland than in normal lactating animals. 2. The mechanical calciphylactic reaction produced by ligating the mammillae is characterized by the precipitation of calcium in the secretion retained in the lumen of the glands. 3. Metabolic calciphylaxis produced by ethionine causes an increase of calcium in the secretion of the glands. It leads to intraepithelial and interlobular calcification of interstitial tissue, the latter exceeds the former. 4. The formal pathogenesis of the two types of calcification is demonstrated and compared with calcifications already known in the human mammary gland.
Zusammenfassung
Vergleichende pathohistologische Untersuchungen an Brustdrüsen des Menschen haben ergeben, daß Kalkablagerungen in retiniertem Drüsensekret, in Epithelzellen und im Stroma auftreten können. Die Pathomorphogenese dieser Gewebsverkalkungen ist weitgehend ungeklärt. Durch experimentelle Untersuchungen sollen pathogenetische Faktoren für die intramammäre Verkalkung aufgezeigt werden. Für das Studium extraossärer Gewebsverkalkungen diente das Modell der Calciphylaxie nach Selye. Die Untersuchungen wurden an 70 Sprague-Dawley-Ratten durchgeführt. Die Mobilisation des Calciumstoffwechsels erfolgte durch einmalige perorale Applikation von 1 mg Dihydrotachysterin (Sensitizer) am 1. Versuchstag. Als zweiter Faktor (Challenger) wurde in einer Versuchsgruppe am 2. Versuchstag durch Ligatur der Zitzen eine experimentelle Milchstauung gesetzt. In einer 2. Versuchsgruppe wurde nach Applikation von Dihydrotachysterin eine metabolische Zellschädigung durch Äthionin (α-Amino-γ-äthylmercaptobuttersäure) durch intraperitoneale Injektion in 2% iger wäßriger Lösung ausgelöst. Gesamtdosis an Äthionin: 400–800 mg/kg Tier. Histochemischer Kalknachweis nach Voigt und v. Kossa.
Ergebnisse. 1. Dihydrotachysterin bewirkt bei lactierenden Ratten eine Steigerung der sekretorischen Aktivität mit Anreicherung von Calcium im Sekret. Auch unter Äthioninwirkung werden stärkere Kalkniederschläge im Drüsensekret beobachtet als bei lactierenden Kontrolltieren. 2. Bei der durch mechanische Sekretstauung hervorgerufenen calciphylaktischen Reaktion ist die Kalkablagerung überwiegend im retinierten Sekret des Drüsenlumens lokalisiert. 3. Bei metabolischer, durch Äthionin bedingter Calciphylaxie entwickelt sich eine Calciumvermehrung im Drüsensekret, eine intraepitheliale und eine quantitativ überwiegende interlobuläre Verkalkung des Mesenchyms. 4. Die formale Pathogenese beider Verkalkungstypen wird aufgezeigt und bekannten Calcifikationen in der Brustdrüse des Menschen gegenübergestellt.
