Summary
The American classification of testicular tumors by Dixon and Moore (1952) is widely used. More recently, a classification proposed by the British authors Collins and Pugh (1964) has gained increased acceptance. In order to determine the practical usefulness of the two classifications, all testicular tumors (278) observed at the Pathological and the Histopathological Institute of the University of Zurich from 1949 to 1970 were reevaluated. Both classifications were easily applicable. The distribution of our cases among the various subgroups agreed well with that reported in the literature. It was found that the classification of Dixon and Moore did not distinguish clearly between the group of embryonal carcinomas and that of teratocarcinomas. In contrast, Collins and Pugh have established useful criteria that permit a distinction of the two tumor forms. Using their system, two independent pathologists reached almost unanimous agreement in diagnostic classification of our cases. The only difficulty encountered had to do with the distinction of “malignant teratoma intermediate type B (M.T.I.B.)” and “malignant teratoma anaplastic (M.T.A.)”. Since both forms of the tumor seem to have a similar prognosis we propose that they be classified within one subgroup. We further suggest that mixed tumors (seminoma and teratoma in the same testis) be classified with the corresponding teratomas. That is justified by the observation these tumors have no essential differences in prognosis. Altogether, we prefer the classification by Collins and Pugh.
Zusammenfassung
Neben der vor allem verwendeten amerikanischen Klassifizierung der Hodentumoren von Dixon u. Moore (1952) hat in den letzten Jahren die Einteilung der britischen Autoren Collins u. Pugh (1964) immer größere Verbreitung gefunden. Wir haben deshalb am Material des Pathologischen und des Histopathologischen Institutes der Universität Zürich die Vor- und Nachteile beider Klassifizierungen geprüft. 278 Fälle von Hodentumoren der letzten 20 Jahre wurden histologisch und katamnestisch kontrolliert. Beide Einteilungen sind relativ leicht anzuwenden. Die Verteilung unserer Fälle auf die einzelnen Untergruppen stimmt auch mit den Mitteilungen in der Literatur gut überein. Bei der Klassifikation nach Dixon u. Moore findet man unterschiedliche Resultate in den Gruppen der embryonalen Carcinome und der Teratome mit embryonalem Carcinom wegen der unklaren Abgrenzung dieser beiden Tumorformen. Bei Collins u. Pugh lassen sich dagegen die einzelnen Teratomformen durch klare Kriterien voneinander unterscheiden. So kamen zwei Untersucher unabhängig voneinander zu einer fast vollständigen Übereinstimmung der Verteilung unserer Fälle auf die verschiedenen Untergruppen der Teratome. Einzig die Abgrenzung der Gruppe „malignes Teratom, intermediäre Form B“ (M.T.I.B.) gegenüber der Gruppe „malignes Teratom, anaplastische Form“ (M.T.A.) bereitete Schwierigkeiten. Da beide Tumorformen eine ähnliche Prognose zu haben scheinen, wäre eine Zusammenlegung zu einer Gruppe wünschenswert. Wir sind ferner der Ansicht, daß die gemischten Tumoren (Seminom mit Teratom im selben Hoden) in die einzelnen Teratomgruppen einbezogen werden sollten, da anscheinend kein wesentlicher Unterschied in der Prognose besteht. Alles in allem ziehen wir die Einteilung von Collins u. Pugh vor.
